Job summary

The Senior Learning Disability and AutismTransformation Manager for SWL & Wandsworth will be responsible for leading on the transformation of both children and young people (CYP) and adult learning disability and autism (LDA) services / workstreams, achieving the ambition set out in the ICB's Joint Forward Plan, the ICP strategy, SWL Mental Health Strategy and the NHS Long Term Plan.

As part of the single SWL LDA transformation team they will be working on the delivery of transformation programmes at Place and will also take a SWL lead role for a LDA programme area / s as agreed with the SWL Head of LDA.

The postholder will work collaboratively with the LDA team to deliver the LDA programme across SWL.







The LDA team is responsible for supporting, monitoring, and providing assurance on the delivery of the SWL LDA, the health inclusion and personal health budget transformation programmes.

The postholder will have responsibility for the delivery of specific programmes of work across these agreed with the SWL Head of LDA.

Main duties of the job

The Senior LDA Transformation Manager - is responsible for :

- Leading on the delivery and implementation of the service redesign and transformation programmes for all age LDA. This will include project planning, implementation, and evaluation.
- Ensuring that the Place priorities for LDA and the transformation approach is aligned to those agreed for SWL as set out in the SWL Joint Forward Plan and confirmed by the SWL LDA strategic Board and Mental Health Partnership Delivery Group (PDG) where relevant.
- The oversight and reporting of LDA KPIs including where Place is identified as the owner such as Annual Health Checks.







Where performance is off track be responsible for flagging the risk to the SWL LDA strategic Board / PDG and leading the development and delivery of any required improvement plans.

- Working with other Place based team members, the provider collaborative, the health and care providers, and partners to ensure that learning disability and autism and LDA services are given the appropriate prioritisation and focus within local Health and Care Plans and other place-based strategies.
- Develop strong relationships with partners at place - health and care providers, the local authority, VCSE and patients and the public.

Ensure that partner roles in the achievement of LDA transformation priorities are clearly defined and progress tracked.

Duties and Responsibilities

Actively engaging and working with colleagues and partners in both Place and acrossSWL to support project delivery.





Working in partnership with colleagues across South West London or Pan London to ensure alignment with other programmes creating joint working to benefit of the whole of SWL.

Chair, facilitate or contribute to multi agency meetings with senior system partners including as required; the chairing of care, education, and treatment reviews (CETR)and Dynamic Support Register (DSR) meetings, and participation in SWL inpatient case surgery meetings and the SWL CETR oversight panel.

Lead the maintenance and oversight of local dynamic support registers (DSR), supported by the LDA team programme support, to ensure effective collaboration between health organisations and local authority partners.

Work with colleagues within ICB, Provider Trusts, Provider Collaborative and LocalAuthorities to support individualised commissioning on occasion when required.







Support the SWL Head of Learning Disabilities and Autism in ensuring that the capacity in the SWL CYP Key worker service is fully utilised and ensuring insights from the SWL Keyworker team, DSRs and CETRs inform strategic transformation.

With the SWL Head of LDA Transformation and SWL deputy Head of LDA ensure that business support model for the team operates effectively.

With agreement with the SWL Head of LDA take the SWL lead for a programme area / s, ensuring that the ambition for the programme across SWL is met by providing leadership and oversight to all parts of the ICS.

Become the ICSs subject matter expert for the programme area, liaising with external centres of excellence as necessary to ensure SWL is receiving best practice guidance to inform SWL delivery and transformation.

Leading on ensuring that Learning Disability and Autism features strongly in publicand patient engagement undertaken at place, working with place team members to achieve this.

Ensure the voice of people with lived experience / experts by experience are at the heart of all LDA programme activities at Place and across SWL.

Leading on the development of any place bids for LDA transformation funding. Working with partners to ensure that Place and the wider ICS is in the best place to secure the maximise the opportunity any transformation funding offers.

Working with the SWL Finance Team to ensure that any LDA transformation money is appropriately accounted for and reported on.

Support the SWL Head and Deputy Head of LDA to meet the team objectives, by ensuring that their portfolio of programmes is planned and managed effectively.

Deputise for the SWL Deputy Head of LDA as necessary.

Person Specification

Education / Qualifications

Essential

- Educated to masters level or equivalent level of experience of working at a senior level in specialist area.
- Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master's level equivalent
- Evidence of continued professional development

Knowledge and Experience

Essential

- Must be able to provide and receive highly complex, sensitive or contentious information, negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups
- Knowledge of legislation, guidance, and policy framework of specialist areas (CYP and adults) including MCA
- Significant experience of successfully operating in a politically sensitive environment
- Problem solving skills and ability to respond to sudden unexpected demands
- Previously responsible for a budget, involved in budget setting and working knowledge of financial processes
- Demonstrated experience in a Healthcare environment
- Successful Project and Programme management experience
- Experience of setting up and implementing internal processes and procedures.

Skills and Abilities

- Ability to analyse complex issues where material is conflicting and drawn from multiple sources;
- Numerate and able to understand complex financial issues combined with deep analytical skills;
- Good attention to detail and ability to identify key points and / or information
- Intermediate (or advanced) Keyboard skill - ability to use Microsoft Office package at intermediate (or advanced) level.
- Comprehensive experience of project principles techniques and tools.
- Ability to prepare and produce concise communications for dissemination to a broad range of stakeholders as required;
- Demonstrated capability to plan over short, medium and long-term timeframes and adjust plans and resource requirements accordingly;
- Ability to make decisions autonomously, when required, on difficult issues, working to tight and often changing timescales
- Proven ability to influence and secure improvement and change across multiple organisations
- Ability to work sensitively, calmly and confidently including with families in challenging situations
