Must be double vaccinated, as it is mandatory for this role. What the Job Involves Ensure compliance with the professional code of conduct and fulfill obligations under relevant legislation. Manage a service caseload, including customers with complex needs, and perform assessments of needs.
Accept referrals, undertake screening, and differentiate between customers' essential and desirable needs. Provide professional supervision to non-qualified staff and discharge statutory duties related to the care of adults and their carers. Develop and maintain effective working relationships with all partners to ensure coordinated, timely, and appropriate services to the customer.
Ensure the safety of both staff and the public by preventing and diffusing difficult situations. Keep up-to-date with developments, products, and information to ensure appropriate and timely information is provided to customers. Ensure proportionate risk assessments are carried out for both customer and carer associated with any support plan.
Undertake reviews of customers with long-term needs as required and carry out reassessments.
