NHS England & NOMS have jointly commissioned a Housing & Accommodation Support Service for male offenders with a personality disorder diagnosis who are on the offender pathway. The service will be delivered via a joint service model including health, criminal justice and specialist third sector housing & accommodation partners. We are looking for Resettlement and Recovery Support Workers to join our team, ideally you will have some experience of working with people with complex issues and knowledge/understanding of personality disorders.

- Knowledge and experience of supporting individuals regarding securing accommodation and developing skills in relation to tenancy sustainment.
- A commitment to reducing repeat offending and improving psychological health & wellbeing.

The role

The main duties of the role are to:

- Develop therapeutic relationships with individuals to support people experiencing a range of emotional states.






- Understand where behaviours are rooted detailed to provide consistent support along with colleagues and professionals.
- Develop and review person centred plans (inc. partners involved in care and support), using planning tools to encourage individuals to be fully involved in support planning.
- Access appropriate NHS services.
- Encourage and motivate individuals to access/participate in socially inclusive opportunities inc. employment, education/training, volunteering; sports & leisure; faith & cultural communities; creative activities.
- Encourage individuals to participate in specialist employment services/ communicate effectively with key partners & statutory agencies and support individuals to liaise with these agencies.
- Support individuals to access debt management,





welfare benefit and legal advice. Managing themselves in their accommodation and maintaining any necessary appointments.
- Deliver person centred support according to support plans, promote 5 ways to wellbeing and support access and registration with NHS services.
- Provide consistent support throughout the transition into independent living.
- Positively manage relationships with others; encouraging individuals to consider the feelings and welfare of other people living in the service.
- Undertake risk assessments and formulate comprehensive risk management plans.
- To be familiar with licence requirements and conditions of discharge.
- To recognise signs of deteriorating mental health and work in partnership to prevent relapse and risk of re-offending.






- Deliver small group activities on site and support external providers delivering programmes and sessions on site.
- Work with private/registered social landlords, Imagine’s housing department, care co-ordinators to assist individuals to secure ongoing accommodation
- Update records and maintain a data base for evaluation purposes.
- To work as an effective team member.
- To ensure that any matter of concern relating to any individual is immediately notified to a manager

Person Specification:

- Minimum NVQ Level 3 Health and Social Care or equivalent or willing to work towards.
- Experience of working with offenders/or people described as having multiple and complex needs or equivalent
- Experience of working with multi agency/multi-disciplinary teams






- Experience of managing a caseload of clients including people with complex needs.
- Experience or working in a community setting in either the voluntary or statutory sector
- Experience of Asset based Approach
- Experience of supporting people to retain and acquire accommodation
- Experience of supporting people into inclusive opportunities
- Excellent communication skills including the ability to engage with individuals experiencing a wide range of multiple and complex needs.
- Ability to assess and manage risk using risk assessment tools
- Ability to communicate with a wide range of professionals from services
- Planning and organisational skills
- Knowledge of the NSF and Mental Health Act/Mental Health Capacity Act






- An understanding of recovery principles in drug and alcohol support
- Knowledge and understanding of Personality Disorder
- Reliable, trustworthy, flexible, responsive and Non-judgemental
- Confident and self-aware, personal resilience, positivity and boundaried
