What the Job Involves
The role involves being a Personal Advisor who will:
Regularly visit and meet with you to discuss your current and future life goals and assist in formulating a supportive Pathway Plan.
Provide information about various opportunities and choices available to you.
Encourage and support your educational aspirations.
Ensure you receive appropriate advice and guidance regarding jobs, careers, and training.
Support your health needs and enquire about your emotional wellbeing as you transition into adulthood.
Organize a Pathway Plan review meeting every six months (minimum) after leaving care to update and refine your plan.
Collaborate with you and significant adults to implement your pathway plan effectively.
Help you develop life skills for independent living and manage a tenancy for independent housing when ready.
Support your participation in community activities and prepare for adult life by providing information on local events and volunteering opportunities.
Provide support up to the age of 25, based on your needs and requests.
