It’s a really exciting time to join us as expand our delivering of the Heathier You NHS Diabetes Prevention Programme throughout South East London and we’re looking for a team of passionate health coaches (or Diabetes Prevention Practitioners) to join us!

The purpose of a health coach is to improve lives,





through empowering people to make effective long-term changes that halt or reverse the progression of Type 2 Diabetes.

This is achieved through successfully delivering our quality programmes and providing evidence-based information, supporting individuals to improve their health, lose weight and ultimately lower their risk of developing diabetes.

Like all roles at Thrive Tribe, you can expect your time at work to be fun, varied and challenging. We don’t like to stick to a firm JD as we know from experience that things change throughout the lifetime of a role and it gives our people the chance to play to their strengths.

A typical day as a Health Coach might include :

- Delivering evidence-based face to face group sessions that focus on diet, nutrition, physical activity and behaviour change across South East London
- Facilitating group interventions in community venues across South East London to support clients in achieving their goals and cultivating a safe and positive environment






- Making service user calls, Triage and onboarding new patients into the programme
- Promoting the Healthier You NHS Diabetes Prevention Programme internally and externally, and improving referral pathways between other services and partner organisations
- Engaging in ongoing training relevant to your role, including submitting your annual CPD portfolio and evidencing professional development in line with our CPD policy
- Working towards criteria and targets established within the National Diabetes Prevention Programmes (NDPP) quality assurance framework, including all Clinical Governance procedures
- Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments we’re always looking to share knowledge and resources!






- Ensuring due consideration and adherence to our internal Thrive Tribe policies, including Information Governance, Safeguarding and Complaints procedures
- Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values!
- The role will require some evening or weekend work.

We’re looking for someone who can combine health improvement knowledge with first class communication and motivational skills.







If you have experience of delivering group-based interventions, and a real understanding of the impact personal characteristics and circumstances can have on health behaviours then we want to hear from you.

Bonus points if you have a qualification in a health promotion related subject, such as nutrition, exercise, or health coaching!

The role involves face to face group session delivery to local people in Bexley, Bromley, Greenwich, Lambeth, Lewisham and Southwark.

Although not essential, so that we can best support the needs of our clients in South East London we are particularly keen to hear from applicants who are fluent in Portuguese, Urdu, Tamil, Spanish, Gujarati, Farsi, Korean and / or British Sign Language for these roles.







We hire based on attitude over traditional skills and experience so if you don’t have all of the necessary experience but feel you are right for the role we would love to hear from you!

increasing every year up to a maximum of 29 days (plus Bank Holidays!)

Contract : Permanent, Full Time (37.5 hours) per week.

Salary : £24,000 per year (rising to £27,000, with development opportunities to earn up to £30,000 as a Senior Health Coach)

There is no office base so the role will require home working, with daily travel across South East London to deliver the group sessions.
