Advanced Clinical Practitioner (ACP) Rapid Diagnostic Centre (RDC) ( Band 8a )

Contract - Fixed Term (1 year). Full-time 37.7 hours per week.

This post will be based at North Middlesex University Hospital (NMUH) to support the RDC team and our clinical lead in maintaining the standards required by the Cancer Alliance.

The post holder will be a key member of the RDC team and will work closely with the Clinical Lead and Service Manager.

Main duties of the job

The role of the Advanced Clinical Practitioner (ACP) is relatively new. Working as part of the multidisciplinary team, they make an important contribution.







The role encompasses undertaking assessment of patients attending our RDC (Rapid Diagnostic Centre) clinics using a range of different assessment and management methods, which may include physical examination, ordering and interpreting diagnostic tests, prescribing medicines (subject to holding a non-medical prescribing qualification) and advanced health needs and to provide high quality clinical assessment and intervention and treatment for a wide variety of patients presenting to NMUH.

To work autonomously at an advanced clinical level in the direct delivery of high-quality care, making critical clinical decisions based on thorough clinical assessment and interpretation of diagnostic interventions and delivery of therapeutic interventions.

The post holder will demonstrate sound clinical decision making and deliver expert care for a wide variety of patients presenting to the NMUH with undifferentiated diagnoses

To work within the pillars of the Multi-professional Framework for Advanced Clinical Practice in England through collaboration with clinical teams to meet the needs of patients,





supporting the delivery and development of service, policy and procedures, and ensuring the maintenance of clinical excellence.

Detailed job description and main responsibilities

1. Assessment and care planning to meet health and wellbeing needs

- Ensure that patients receive high quality clinical care including formulating a working medical diagnosis for patients sometimes presenting with undifferentiated urgent or emergency health care
- Ordering and interpreting diagnostic tests.
- Request initial laboratory and radiological tests as required
- To work competently in the role of Advanced Critical Care Practitioner, being aware of the boundaries of the role,





referring patients appropriately to other healthcare professionals as
- Making referrals to other health care
- Prescribing medicines / PGDs and
- Ensure that effective discharge planning and health promotion is
- To monitor quality and continuity of patient care and ensure standards are
- Ensure accurate, contemporaneous and comprehensive
- Establish and maintain effective communication with nursing, medical and paramedical colleagues to provide optimum patient
- Communicate effectively with patients, relatives, and
- Ensure patients are informed and consent is gained prior to treatment, investigation and management as
- Participate in nursing audit, taking action to address improvements
- To provide expert nursing / medical advice and support to patients,





their families and other healthcare professionals following diagnosis and through
- To liaise with local community, social services, paramedics and voluntary organisations in order to develop existing services for clients, carers and health
- To liaise with the MDT and outside agencies to ensure appropriate and timely discharge / follow-up arrangement in place for
- To actively involve service users in providing feedback of their experience of the current service and suggestions for
- Actively implement effective systems and processes for infection, prevention and control management and relevant audit within their clinical
- To accurately identify those vulnerable adults at risk of abuse

2. Managerial responsibilities :







- Contribute to the review and development of clinical patient pathways to ensure standardised approach to emergency
- Lead on developments within the service
- Participate in the recruitment and orientation of new staff
- Take appropriate action in managing incidents relating to violence / aggression within the
- Take appropriate action in dealing with situations that may result in a disciplinary action.
- Participate in the management and development of the clinical service with the
- Participate in the investigation of Adverse Incidents and
- To play an integral role in the operational and strategic development of specialist
- Liaise effectively with all other disciplines within in the Trust and at local






- Initiate and participate in management of change to broaden activities, standards and communication links through the acute trust and primary
- To actively contribute to any relevant initiatives within the care group and provide support to senior colleagues and head of
- Act as an effective role
- To work with and support managers and clinical staff to identify areas where changes in practice are
- Ensure the formal reporting and recoding of adverse incidents in line with Trust policy.
- Network with the team of clinical nurse / AHP specialists within the Trust to evaluate, develop and promote specialist-nursing and AHP
- To utilise and maintain information systems to aid required audit and to provide regular actively analysis
- To maintain records and send reports (e.g. police reports)
