Responsibilities :
- Conducting thorough assessments under the Care Act to determine patients’ needs and support requirements.
- Experience Coordinating services and resources to ensure smooth transitions for patients leaving the hospital.
- Able to demonstrate expertise in adult social care legislation and practices.
What we are looking for in a Social Worker :
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
- Degree level or equivalent in Social Work
