JobSummary :
Liverpool EarlyHelp utilises a strength based approach to provide empoweringinterventions to children and families to help address emergingworries and affect positive change to improve the longer termoutcomes of children and young people.
Asthe lead professional you will coordinate support with ourmultiagency partners to ensure that a whole family approach istaken to address the identified needs of the family.
Weare looking to recruit an Outreach family support worker toundertake early help assessments with families create support plansand chair team around the family meetings to review the progressand identify any additional areas of support.
Therole involves direct work with children and young people to exploretheir views and worries to ensure that the plans / interventions arehelping to improve theiroutcomes. .
KeyDuties / Accountabilities
Taking wholecase responsibility to provide high quality outreach and homebasedEarly Help support to children and families using a whole familyapproach (level 2 and3).
Deliveringtargeted support to children across age ranges (0 25*) and theirfamilies in order to prevent the need for statutory services and toensure families access Universalservices.
Between ages1925 relates only to Adults where the SEND Code of Practice isapplicable.
Act as caseholders for complex cases that step down from childrens social care(level 3 / 2).
This willinvolve completing early help assessments arranging Team Around theFamily meetings and supporting families to improve outcomes fortheir children by taking on the role of the Lead Professional andassociatedtasks.
Offer supporton cases that become more complex and reach level 4.
Support iscontinued on these cases to allow a smooth transition between earlyhelp and socialcare.
Provideintensive and targeted support to vulnerable children and youngpeople. This includes bespoke packages and programmes.
This alsoincludes supporting adolescents with complex emotional and mentalhealthproblems.
EssentialExperienceRequired :
Experience ofdirect work with children young people and their families isessential.
Experience ofeffective collaborative and multiagency working to support childrenand families isessential.
Experience ofdelivering outreach and home based support to children and familiesto address areas of identified need isessential.
EssentialQualificationRequired :
NVQLevel 3 in childcare and education NND Health and Social Carequalification isessential.
Additionalinformation tonote :
Working hours : 35 hours perweek.
The rolerequires an Enhanced DBScheck.
