The job

You will be working with our clinical teams to deliver the best primary care healthcare service to our patients. More specifically, clinical responsibilities include:

Performing both new patient reviews and follow-up consultations for patients who require input about their mental health. All reviews will encompass a thorough assessment,





formulation of the issues and produce an objective management plan for the patient. This will include care plans where appropriate, especially for patients who have presented for multiple review appointments.
Helping people experiencing mental health problems to make informed choices about pharmacological and psychological treatment options, by providing education and information on the benefits and unwanted effects, choices and alternatives. They must support people to identify actions that promote health and help to balance benefits and unwanted effects.
Liaising with other agencies where required who may be involved in the patient's care - for example, community mental health teams, crisis mental health teams, other primary care practitioners for example at the PCN level.
Recognising and responding to the needs of all people who come into their care including young people, pregnant and postnatal women, people with physical health problems, people with physical disabilities, people with learning disabilities,





older people, and people with long term problems such as cognitive impairment.
Working positively and proactively with people who are at risk of suicide or self-harm, and use evidence-based models of suicide prevention, intervention and harm reduction to minimise risk.
Ensuring patients on a mental health register (for example, depression register or SMI register) are reviewed appropriately to ensure clinical targets are met regarding mental and physical health needs. This includes performing the appropriate physical health checks for patients on these registers. Nurses will be trained appropriately and should be able to do this without supervision.






Ensuring information regarding patient needs and progress are communications promptly and accurately to other people involved in their care, for example other primary care practitioners or secondary care colleagues. Ensure that patient information is shared having due regard for confidentiality.
Facilitating flow through the service to ensure that appropriate cases are referred onwards/escalated to appropriate service levels including our General Practitioners.
Utilising all available resources, for example those shared on the Penrose Portal, so that patients' care is delivered following Penrose Health policy requirements.
Discussing clinical case management with their direct supervisor should there be any ambiguity about a formulation or treatment plan for a patient.





This will be performed promptly so that the patient is still receiving timely diagnosis and management.
Performing and completing referrals to other mental health practitioners - for example, referrals to secondary service providers. Should this be for diagnosis and/or more complex management/treatment plans
Delivering depot medication for patients who require long-term intramuscular injections, as directed by shared care agreements.
Maintaining high standards of care, working with service users to ensure their individuality and dignity is maintained at all times, while promoting independence.

About you

The right candidate will demonstrate best-in-class interpersonal and communication skills for dealing with our diverse patient population, and be able to work well in a close-knit team of both clinicians and non-clinicians. Strong clinical knowledge and high levels of empathy and tolerance are important to ensure successful consultations.

Minimum requirements: You must have a current NMC registration, and we would ideally be looking for someone with experience working in a GP Primary Care setting (although this is not required). Previous experience working with EMIS is also preferred but not required.
