Care Coordinator - Job description
Comprehensive Care LTD is looking for a Care Coordinator to join our fantastic Care team based in Failsworth Manchester.
You will take on new packages of care, support in preparing rotas, take feedback from families and service users 8 weekly, carry out spot checks on monthly basis, liaise with social workers / family members to mentoring and supervising care workers, supportt the rota and on call system. Our Care Coordinators work closely with the Supervisor team, Care Manager and Registered Manager to ensure we continuously provide an excellent, compliant, and quality service.
Care Coordinator - Some of the key responsibilities involved in the role:
- Developing good working relationships with external stakeholders
- Supporting the team with growth and spot checks
- Setting up new packages of care
- Producing rotas for care workers
- Monitoring the effectiveness of the service and on call number
- Handling and recording complaints and compliments
- Providing on-going support to service users, their families, and friends
- Monitor,
supervise, support and mentor our care team
- Ensure accuracy and completeness of all documentation, written reports and administration is completed to a high standard and within a timely manner
- Deliver personal and non-personal care to customers if necessary
- This is an ideal opportunity for an experienced Care Coordinator ideally with a professional training qualification with NVQ/QCF level 2 or 3 in Health & Social Care etc.
Essential requirements:
- A good level of written and spoken English
- Be prepared to undertake training as required
- Good communication skills
- Supporting staff in delivering targeted work with individuals to plan their support and achieve the goals and outcomes that are important to them
- Contributing to the ongoing development of team working and listening, sharing and responding to feedback from people who use the service.
- Providing support and advice to staff around safeguarding issues and work closely with Service Manger on complex issues.
- Ensuring all clients received a high quality service and that all actions were carried out by day services.
