Description

We are looking for a Transition Officer to manage and lead transitions from schools to college, from course to course and post further education, working closely with Heads of schools, SENCO’s and external stakeholders locally across Kent and Medway.

The successful candidate will be representing the College, ensuring that students are offered and experience high quality, impactful learning support, contributing to the delivery of an efficient statutory SEND service as detailed in the Children & Families Act (2014) and the SEND Code of Practice (2015), leading on transition planning between schools and agencies to prepare and meet the students’ needs and ensure successful transition into College.







Main Responsibilities and Duties :

1. Develop and sustain professional working relationships with all local feeder schools, mainly Heads of Schools and SENCO’s, stakeholders and, parents / carers acting as a key link in order to implement and manage the transition arrangements for students with additional needs.

2. Develop and co-ordinate 14-16 School Partnership links, overseeing the delivery and quality of service provision.

3. Meet the statutory responsibilities of the Local Authorities, including those related to the Children and Families Act 2014 and the monitoring of curriculum and other provision made under the direction of the EHCP.

4. Liaise with SEN Practitioner and ALS Funding & Contracts Manager to ensure EHCP learners are identified through transitional work with schools.

5. Produce Transition and Support plans for EHCP learners, participating in the Annual Review process for EHCP students, acting as the Key liaison with curriculum to ensure consistent implementation,





review and update of support plans.

6. Deliver Transition programmes with individual / groups of learners in the lead up to enrolment.

7. Develop and deliver external training with stakeholders to maintain links, attending events offsite to further develop links within the community.

8. Identify trends and needs in order to inform and develop support for students, focusing particularly on transition and retention initiatives that help students settle and stay in college.

9. Maintain accurate and up to date records to assess outcomes and to examine the impact of interventions and initiatives.

10. Work co-operatively with other members of the college and external agencies and stakeholders to make recommendations to impact on the wellbeing of students.







All staff are required to :

- Complete mandatory annual Continuing Professional Development (CPD) in accordance with the number of hours set for their Department.
- Travel between sites as and when required in accordance with their job role.
- Undertake occasional evening and weekend work to meet the needs of the service.
- Comply with and understand all aspects of legislation and College policies and processes relating to safeguarding, including promoting the welfare of children, young people and vulnerable adults.
- Form and maintain appropriate relationships and personal boundaries with children and young people.
- Comply with,





understand and promote statutory and college best practice in Health & Safety at a level relevant to the role and responsibilities held by the post holder.

- Have access to confidential information which should not be disclosed to any other person unless in pursuit of your duties in compliance with GDPR or with the specific permission given on behalf of the College.
- Actively take responsibility for their personal learning and development (informal and formal) reviewing and reflecting on their performance within their current role.
- Promote, adhere to, understand and put into practice all college policies and procedures.
