This is an exciting new opportunity within theIntegrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service.

We are seeking a highly motivated band Nurse with a special interest in Stroke. The Integrated service provides both stroke and community neuro rehabilitation, supporting service users with a variety of complex needs.

This role requires the successful candidate to work in a clinical lead capacity in the service, developing stroke pathways but will also hold a clinical caseload undertaking specialist assessments of patients with complex and multiple pathologies using advanced, specialist clinical reasoning skills.

In addition,





the successful candidate will work in partnership with the existing ABI Coordinator to ensure the smooth running of the Level ABI Pathway.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle.

You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.

Main duties of the job

You will use your clinical skills and knowledge to shape the service, support others in their development. You will work closely with all members of the Multidisciplinary Team and assist the Locality Team Lead with service development.

You will be involved as needed in the complaints process and serious incidents reviews.







You will provide comprehensive assessments, identifying the patient needs, agree a treatment plan and provide appropriate interventions relating to their physical, functional, psychological and cognitive needs.

You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.

The successful post holder will receive and provide clinical supervision and an annual appraisal.

The ideal candidate should be flexible and adaptable to support service delivery.

The successful post holder must be able to travel independently across their work locality,





have a valid driving licence and access to a car for work purposes.
