This post is a secondment opportunity, prior permission from candidates line manager will be needed.
The post holder will support the delivery of safe and effective patient services by facilitating the provision of the most recent clinically effect researched based, cost-effective tissue viability/chronic wound advice to trust staff and primary care for complex cases.
A key aspect of the role will be monitoring the effectiveness of tissues viability/chronic wound interventions, provide specialist advise of best practice relating to all aspects of tissue Viability in line with both local and national guidance to al staff, patients, and carers.
To identify training needs and provide training in chronic wound management.
To maintain close links with relevant services in primary and secondary care settings to ensure clients receive a seamless service.
A collaborative approach is essential working within the multi-disciplinary team to provide comprehensive patient focused care.
The post holder should demonstrate an awareness of local and national nursing agendas.
- Provide expert advisory and consultative services to health care professionals across community and primary care and secondary care settings.
- Provide support, supervision and education to clients, carer's and relatives with specialist care needs.
- Provide support and guidance within the interdisciplinary team to effect the modernisation of specialist care services.
- Have responsibility for the assessment of care needs and the implementation and evaluation of client care for a specific caseload.
- Facilitate a pathway of care between primary and secondary care settings by developing strong links with secondary and primary care to provide a seamless service for people with specialist needs.
- Participate in teaching and developing all other interdisciplinary staff and statutory, voluntary agencies about tissue viability using a variety of effective and comprehensive methods.
