Social Worker, Young Adults Transitions Team, Bristol
Pertemps are recruiting for a qualified and experienced Social Worker to join the Transitions team. The postholder will lead the way to ensure the best possible outcomes for young people aged 14-25 years in the service.
They will support completion of Care Act assessments and support plans leading to the commissioning of services to meet identified need where non-commissioned support, Technology, equipment, or wider community assets could not ensure independence outcomes.
Pay rate competitive
The successful candidate must have prior experience within adult social care, with experience in legislation specific to Transitions.
