Job description
An exciting job role has recently become available at the Stockport Metropolitan Borough Council. They are looking for a new member for their Adult / Hospital Discharge Team.

Requirements :
- Join a newly created all-age adult mental health hospital discharge team, supporting older and working-age adults with mental health diagnoses.
- Collaborate with Pennine Care colleagues to improve mental health services and outcomes for individuals leaving the hospital.
- Ensure safe and timely discharge from hospital for individuals with functional or organic mental health conditions.
- Work collaboratively with individuals, carers, health professionals, advocates, and third-sector colleagues.
- Co-produce Care Act assessments, support plans, and risk assessments.
- Conduct Mental Capacity Assessments and chair Best Interests Meetings.
- Contribute to improving the discharge process through teamwork and partnership.
What we are looking for in a Social Worker :
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
- Degree level or equivalent in Social Work
