Social Worker, Mental Health Team

Social Worker required for Sheffeild Council's Mental Health Team. Immediate start with ongoing contract available with competitive rates up to £30.00 per hour

Responsibilities include:

- Support, care planning, mental health and CPA reviews
- Safeguarding Comprehensive needs assessment
- Risk assessment and management of mental health symptoms
- Ensure accurate and timely recording of all information/assessment activity onto the social care records database
- Be a skilled and reflective social work practitioner with demonstrable experience of working with severe mental illness in a community setting.
- Have knowledge and experience of working within as a care coordinator in a local authority or the NHS.






- Care co-ordinate a caseload of service users with complex needs; assess new referrals including safeguarding and vulnerable adult referrals

Minimum Requirements:

- SWE Registration
- 2 year post qualifying experience within UK
- Eligibility to work in the UK
