Job summary

The postholder will be an autonomous practitioner working independently and inconjunction with other health care professionals to assess, diagnose and treatthe conditions of patients attending within the i-Heart service initiatingdirect referrals as appropriate.

Asa non-medical prescriber your duties will include visit triage, home visits tohousebound patients and seeing on the day patients for acute and minor illnessin a clinical setting, working on behalf of Barnsley Healthcare Federationproviding a service to Barnsley PCN practices.

You will work as part of amulti-skilled, professional team alongside GPs, Physician Associates and otherACPs. Provide expert professional advice to patients,





carers and colleagues andensure the maintenance of clinical excellence.

Provide new and innovativeconcepts, models, methods, and practices to deliver new and improved i-Heartservices. Support with education and training to other staff and students and undertakeresearch as part of the role.

Main duties of the job

UndertakesClinical Practice at an advanced level using expert knowledge and clinicalskills to deliver holistic care to people accessing the iHeart service.

Assessesdiagnoses and treat patients attending the iHeart service with a range ofacute, non-acute and chronic medical conditions.

Assessesdiagnoses and treats patients in surgery, hubs or own home who require acutemedical attention and refer to appropriate agency.

Directlyadmits patients to secondary care hospital in acute medical need.

Referspatients to other medical specialities for assessment if deemed appropriate.

Referspatients as appropriate to other members of the multidisciplinary team,





secondarycare and to other statutory and voluntary organisations including referral forx-ray.

Hasundertaken x-ray training as required by hospitals trust.

Can interpreta range of diagnostic tests and perform routine clinical procedures cytology, venepuncture, blood pressure monitoring etc.

Makescritical judgements of the highest order to satisfy the expectations anddemands of the job first contact with patients presenting acutely andmanages care in the presence and absence of protocols.

Makesdecisions where precedents do not exist, where appropriate without recourse toothers.

Advisesand supports others where standard protocols do not apply.

CAREAND PROGRAMME MANAGEMENT

Develops and manages highly specialised programmes of care and care packagesand provides highly specialised advice concerning care normally beyond thescope of normal Nursing practice establishes differential diagnoses,management / treatment plans and makes referrals as appropriate.

Makes fast trackreferrals.

Communicates highly sensitive condition related information to patients,relatives and carers.







Initiates the process of diagnosis with patients suspected to have a chronicdisease Diabetes, COPD, asthma, IHD referring to other clinical staff asappropriate.

Undertakes annual review of patients understanding and ability to self-manage.

Refers toother members of the extended primary health care team as appropriate chiropodist,dietician, specialist Nurses.

Takes part inclinical audits.

STRATEGIC DEVELOPMENT OF PRIMARY CARE SERVICES

Contributesto the strategic development of i-Heart and additional public health services tomeet patient and population needs within the practice and across the widerorganisation.

Works / liaises across multi-professional and agency boundaries.

Determineshow to deal with ambiguous or unique problems to develop and advance i-Heart servicesfor the benefit of patients,





carers and communities.

Recognition andinvolvement in the Federation as a teaching unit and help drive the Nursingelement of training.

Developlinks within the Federation and other local and national bodies relevant to i-HeartNursing initiatives.

HEALTHIMPROVEMENT

Initiates and carries out programmes of health screening and ensures other teammembers are aware of health agendas and screening programmes.

Introduces innovation in i-Heart to meet on going health need demands of thepractice.

Works to introduce new developments that will improve health outcomes forpatients and is able to demonstrate through written and presentation thesedevelopments Sexual Health agenda.

Shares innovation with others.







Supports practitioners in profiling the practice population in order toinitiate contribute and promote improved health and prevent disease inindividuals and groups.

Represents individuals and families interests when they are not able to do sothemselves.

Makes referrals to statutory and voluntary agencies as appropriate.

COMMUNICATION

Provides and receives highly complex, sensitive or contentious information andfrequently deals with situations which are hostile or emotive.

Uses developed communication, negotiation, conflict management skills whichrequire empathy, reassurance and persuasive skills where there can besignificant barriers to acceptance which need to be overcome.

Regularlycommunicates service related information to Partners, Business Manager,Salaried GPs, Nursing and Administrative staff, patients, relatives andcarers.

Builds and communicates therapeutic working relationships with a wide array ofstatutory and voluntary organisations for the benefit of patient care andfacilitates good working relationships.

Develops and delivers presentations to large groups local and nationalconferences, clinical meetings, empowers staff to develop presentation skillsin order to promote good practice.

Develops a leadership role within the i-Heart service.

Encourage and motivate others to be forward thinking in ideas that address thehealth needs of the practice that will be innovative whilst managing anddeveloping new ways of working, Implanon clinic, teenage health clinic.

Partakes in clinical supervision for own development.

Actively participates in clinical and practice meetings.

Develops own knowledge and practice to meet objectives / changes in service.

Integrates theory into practice and establishes formal links with educationalinstitutions.

Assists in the mentorship and assessment of trainee Nurse Practitioners.

Acts as mentor for pre-registration students and other staff within the i-Heartservice.

Leads on setting standards and promotes best practice.

INFLUENCE ANDDEVELOP POLICY

Responsible for establishing how policies should be interpreted and translatedwithin the practice.

Implements local / national, PCT and practice policies, proposes developments andimprovements to service delivery.

MANAGE AND USEINFORMATION

Makes effective use of the clinical system by ensuring the consistent andaccurate entering of data and use of Read Codes.

Facilitates the use of up-to-date information technology in order to collateaccurate and timely information as and when required by the i-Heart service.

Ensures a satisfactory call and recall system of patients.

Assist in the development of practice protocols for the surgery and clinicalprotocols for the clinical team.

Identifies, monitors and review

reviewsdeficiencies in procedures and implements remedial action.

RESEARCH ANDDEVELOPMENT

Continuous involvement in research / evaluation and / or development work which maybe as part of one or more formal research programmes or activities.

Actively contributes to Practice / Service / Team audit / Research activity asrequired.

Instigates audit activity as required new services.

Identifies, applies and disseminates research findings relating to clinicalpractice.

WORKINGCONDITIONS

Frequent exposure to body fluids, blood, wounds, fleas and lice.

Occasional exposure to aggressive behaviour

Frequent use of VDU equipment.

Frequent requirement to travel between different locations.

Person Specification

Qualifications

Essential

- ACP / RGN / RN Qualified.
- Masters level (or working towards) in Advanced Practice Studies, plus supplementary post graduate level courses : Extended and Supplementary Prescriber.

Desirable

- Teaching and assessing in clinical practice.
- Counselling skills.
- Minimum of 3 years post Advanced Practice experience within the clinical field.

Experience

Essential

- Demonstrate the application of evidenced based practice.
- Able to demonstrate a depth and breadth of current issues and policies within the NHS and have the ability to translate into practice.
- Experience in leadership role and change management.
- Autonomous practitioner working independently and in conjunction with other health care professionals.
- Very high level of clinical skills and competency relevant to the role.
- Understanding and experience of interagency working and adult / child protection.
- Evidence of continued professional development.
- Experience of Clinical Governance and risk assessments.

Training and Education

- Willingness to undergo education / training for both practice and service needs.
