Job summary

The post holder will act as an expert Mental Health / Learning Disability Nurse providing specialist clinical and care management advice and support for adults with a diagnosis of a learning disability who have complex needs and for whom, North Central London (London Boroughs of Barnet, Camden, Enfield, Haringey and Islington) has commissioning responsibility.

The post holder will develop positive working relationships with networks of health and care professionals, working in a person-centered way to support the wellbeing of the clients.

The post holder may support clients with fully health funded, or joint health and social care needs who require specialist health input.







They will be actively involved in developing care plans and advising on appropriate packages of care in the community or supporting with stepping down from an inpatient mental health facility.

They will ensure the delivery of high quality that is effective, safe, and ensure that client's experience of care is positive.

Main duties of the job

Good understanding of the commissioning principles to ensure that packages are commissioned based on need and in line with the ICB's principles.

Plan and organise their own day to day work, be independent and keep clear records of actions.

Collaborate with the commissioning managers to contribute to the saving targets via their input regarding needs-based commissioning decisions.

Effectively manage a caseload of clients with a mental health / learning disabilities and complex care needs.Contribute to planning, implementation and evaluation of care plans using evidence based and person-centred principles.

Liaise and work in a joined-up way with the client, their family,





relevant health and care professionals, and all other health and social care agencies involved in the care of the person.

Represent the ICB in complex care panels and Section 117 panels to provide clinical oversight on care and risk management plans, and help inform discussions effecting resource allocation, including making recommendations to commissioners on what care should be commissioned to support that individual to meet their needs.

Complete quality assurance assessments and monitor community the quality of care offered from inpatient providers the ICB commissions for individuals with complex care needs (learning disabilities and mental health).

Person Specification

Education / Qualifications

Essential

- Valid professional registration with MNC
- Clinical experience in mental health and / or learning disabilities
- Evidence of CPD (list and examples are sufficient, don't need to see copies of actual certificates)

Experience

Essential

- Full understanding of Section status relating to mental health patients (S2 / S3 / S37 / 41 / S117) and the impact on service delivery and funding streams - examples of case management and relationship development with clients and other MDT (multidisciplinary team) members - devise, implement and review section 117 aftercare plans - discharge clients from s117 following needs assessments






- Awareness of NHS funded care and how its application is in practice. Examples : assessing care packages for clients under section 117 aftercare by implementing the value for money principle and following a thorough needs assessments

Desirable

Knowledge of the CPA / CTR processes and of the Community Care Act Examples : C(E)TR (care (education) and treatment reviews), CPA (care programme approach), DSR (dynamic support register), Transforming Care Programme, Care Act, CHC (continuing health care) framework

Knowledge / Skills / Abilities

Essential

- Ability to work in partnership and liaise with a range of health and care professionals involved in clients' care. Examples : Local Authorities, care providers, hospitals, clients / families






- Ability to communicate and adapt communication style to suit a range of audiences clients, carers, commissioners, care staff etc.

Includes feeling confident to challenge when required.

- Ability to handle distressing situations with sensitivity and empathy and show resilience when dealing with stressful or upsetting situations
- Ability to work autonomously and take on initiatives when required
- Ability to present assessments justifying outcomes
- Ability to move quickly between tasks, often multitasking or managing a few urgent issues in a day
- Commitment to the values and principles of system working agreed across NCL
