Job summary

Intermediate Care Southwark

Intermediate Care Southwark offers domiciliary-based rehabilitation and support for clients, who are 18 years and over, to maximise independence.

We treat clients with a wide range of functional difficulties, long - term conditions and disabilities.

Intermediate Care Southwark is an integrated partnership across health and social care and uses an enabling approach and goal orientated rehabilitation in order to maximise a client's independent living skills in order to reduce the need for long-term care.

Occupational Therapists will work as part of the wider MDT, comprising of Physiotherapists, Nurses, Pharmacy, Social Workers, Assistant Practitioners,





Rehabilitation Support Workers, Senior Rehabilitation Support Workers, Assistant Practitioner OTs and a Specialist Geriatric Consultant.

As a Senior Specialist Occupational Therapist in the team, you will have an opportunity to :

oDevelop your skills in complex and holistic case management of clients beyond traditional Occupational Therapy practiceoFurther develop your clinical skills within Occupational Therapy assessment and rehabilitation for clients with a range of conditions and presentations.

oSupervise and develop junior staff, both as part of formal supervision, and with clinical oversight and training to the wider team.

oDevelop your leadership and organisational management skills.

You will be supported by the Clinical Specialist Occupational Therapist, and will have access to training from within Guy's and St Thomas NHS trust,





as provided by Southwark Council and be able to apply for funding to complete external courses which are applicable to your role and will help to progress you further within your career.

Job responsibilities

Main duties of the job :

- To be responsible for providing high quality evidence based rehabilitation to clients with a wide range of complex disabilities and needs, who may have multiple pathologies and co-morbidities.
- To be responsible for the management, co-ordination and provision of specific areas of service delivery within Intermediate Care Southwark.
- To be a source of clinical expertise and advice on Occupational Therapy client care for Occupational Therapy colleagues, other health professionals within the multi-disciplinary team and professionals working in external organisations.






- Provide supervision and facilitate professional development of Specialist Occupational Therapists, Rehabilitation Support Workers and Occupational Therapy students as required.
- To work as an independent, autonomous and frequently lone practitioner in the delivery of client care in the community setting.
- To work as part of a flexible multidisciplinary team, across traditional professional boundaries to meet the needs of clients using a holistic and client centred approach
- To support the departments commitment to meeting patient needs and delivering excellence it is essential the post holder be able to work their hours flexibly and contribute fully to both existing and new rotas over a 7 day week, including weekday and weekend on call shifts







The post-holder will be expected to deliver at least 50% of direct clinical care as part of the role.

Essential

- oAbility to produce clear and concise reports
- oAbility to devise, deliver and evaluate training both within the service and to other agencies
- oAbility to demonstrate skills sharing and collaborative multi- professional working, at a variety of levels, including ability to co-ordinate multi-disciplinary / multi- agency meetings
- oAbility to carry out a range of risk assessments, in a variety of community settings
- oAbility to work effectively under pressure with frequent interruptions
- oAbility to plan and organize the provision of care to teams caseload of patients

Skills

Essential







- oAbility to manage a complex and constantly changing workload of clinical and non-clinical duties demonstrating excellent time management, prioritisation and organisational skills
- oAbility to organize and plan staff rotas and make best use of available staff resource
- oExcellent interpersonal and negotiating skills, including observation, listening and empathy skills
- oAble to develop effective therapeutic and working relationships, demonstrating empathy and motivational skills
- oDemonstrate highly developed verbal, non-verbal and written communication skills in English

Desirable

oDemonstrate effective research and IT skills

Qualifications / Knowledge

Essential

- oRecognised Occupational Therapy degree qualification or equivalent






- oHealth and Care Professions Council (HCPC) registration
- oExperience of working with adults and older people with chronic conditions and complex co-morbidities
- oRehabilitation experience at minimum Band 6 in all core areas : neurology, orthopaedics, respiratory, and care of the elderly
- oAbility to use an evidence based approach including clinical outcome measures to evaluate outcomes
- oAbility to work as an autonomous practitioner and to make sound clinical judgments and decisions regarding patient care and to be responsible and accountable for decisions made

Desirable

- oMember of relevant Clinical Interest Group Older People, AGILE
- oRecognised post-graduate qualification in specialist clinical area
