Opportunities have arisen for registered professionals - Band 6 to work flexibly within the Intermediate Care at Home and Discharge To Assess team.

The Discharge To Assess element is a multi-professional service who assess patients upon discharge from hospital in the community .

We provide individualised care to meet the person's needs and facilitate transfer for onward care into the appropriate service : Health,





Adult Social care or partner agencies.

Main duties of the job

The Discharge to Assess element : to support people who are clinically optimised and do not require an acute hospital bed, but may still require care services with short term, funded support to be discharged to their own home or another community setting.

Assessment for longer-term care and support needs is then undertaken in the most appropriate setting and at the right time for the person.

We provide holistic multidisciplinary interventions and support in order to stabilise patients in their own home or usual residence.

This comprises a multi-disciplinary integrated team.

The Rehabilitation Element incorporates a free, short-term rehabilitation service for adults to help prevent unnecessary hospital admission, enable early discharge from hospital and prevent unnecessary admission to long-term residential care.







This role will require the post holder to be flexible to cover all areas within the Active Recovery and Discharge to Assess service covering 7 days

Job description

Job responsibilities

The post holder will be a qualified HCPC registered Physiotherapist. You will be working within a multi-professional team incorporating physiotherapists, occupational therapists, nurses, social workers and rehabilitation support workers, as well as liaising with other community health and social care teams.

The whole service provides a person centred approach, which can be provided in a clients own home or in a designated residential care service is provided across 7 days a week covering core hours of 8 am 9 pm.

You will be based at Regent House, Heaton Lane, Stockport, SK4 successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team.

You will be offered 1 :





1s and access to supervision peer support. The service promotes regular profession specific and MDT team meetings.

You will need to be a registered professional with an enthusiasm for working within the community with the ability to work flexibly between all aspects of the service.

We place a great emphasis on training and personal development and will provide you with access to training and supervision.

The candidate will be able to demonstrate sound clinical reasoning, excellent time management and advanced communication will be support available from clinical leads.

Lone working is an expected element within this community role with support from the Multidisciplinary team.

If successful this vacancy will require an enhanced DBS check at a cost of £ This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.

Person Specification

Education and Qualifications

Essential

- oDipCOT / Degree in Physiotherapy
- oState Registration with the Health and Care Professions Council.






- oUK driving licence (will need use of a car for work purposes and business use insurance on commencement of post). Driver with daily access to a car
- oEvidence of Continuous Professional Development maintained in a portfolio including attendance at recent postgraduate courses relevant to the clinical fields in rotations

Desirable

Related academic attainment

Knowledge

Essential

- Knowledge of Physiotherapy issues relating to a wide range of conditions- theoretical, practical and professional
- Knowledge of the principles of clinical governance
- Health, safety and risk awareness. Knowledge of Risk Assessment and management.
- Understanding of professional ethics and their application and practice.
- Knowledge of health legislation and current practice.





Knowledge of Mental Capacity Act and application
- Knowledge of Safeguarding responsibilities
- Evidence of clinical reasoning

Desirable

- oKnowledge of working of community Physiotherapy and intermediate tier services
- oCompletion of recognised relevant postgraduate courses.

Experience

Essential

- oExperience of working as part of a therapy team and multidisciplinary team.
- oExperience of student or assistant supervision and clinical education of less experienced staff or students or assistants.
- oExperience of working with patients with dementing illness, confused patients and patients with severe physical limitations to mobility.
- Experience of participation in complex discharge planning

- oExperience of community working
- Experience of conflict resolution
