This is your chance to join First Community Health and Care, a staff owned social enterprise providing CQC Good frontline NHS services in East Surrey.

We are looking for a Band 5 Staff Nurse with the enthusiasm and flexibility to thrive on providing exceptional quality patient focused care in a community setting.

Its a role that offers great flexibility, but also requires some flexibility from you plus a valid driving licence and a suitable car.

As part of the local NHS, you will play a vital role to reduce NHS system pressure.





We deliver high-quality nursing, specialist care teams, children and family advice and support, plus our rehabilitation ward and Minor Injury Unit at Caterham Dene Hospital.

To fit around you, we can offer a range of full / part time hours and shift patterns including day, late and evening shifts.

We will also consider bank (minimum of hours). There is a welcome benefit of £2,000 available for this role (terms and conditions apply).

This role is suitable for both experienced and newlyregistered nurses. We have a supportive Preceptorship Programme for new nursingregistrants.

Main duties of the job

Based in Caterham, Oxted and Redhill, you will provide nursing care for a wide range of conditions to people in their own home or residential settings.

Responsible for assessing, planning,





implementing and evaluating individualised care under the direction of a Clinical Team Lead and District Nurse, you will work closely with health and social care professionals across the primary care networks to prevent your patients being admitted to hospital.

Job responsibilities

Job Summary

Thisrole within First Community (FC) is to provide nursing support to adults andtheir families. You will deliver skilled nursing care under the direction ofthe Senior Nurse within the framework of the NMC Code, promoting a healthylifestyle and independence where possible.

You may take responsibility foroverseeing the nursing care within your area in the absence of the SeniorNurse.







Key responsibilities / outcomes

- To work within a rota to cover the needs of theservice, 52 weeks of the year.
- To plan and monitor the delivery of person centredhealth care, evidencing clinical decision making in the clinical record.
- To participate in the admission and dischargingplanning of patients from the caseload.
- To promote the wellbeing, health promotion andself-management of patients health needs.
- To work within FC and local policies, guidelines,procedures and SOPs.
- To promote and ensure effective communication,working within FCs behaviour framework.
- To supervise and delegate care provided by morejunior staff, while maintaining accountability.
- To recognise and escalate any concerns to thesenior nurse.






- To maintain records in accordance with FCs policyand NMC Guidelines, including electronic patient and data management systems (EMIS).
- You will be able to work independently and as amember of the team, working in partnership with the wider Health and SocialCare Networks.
- To maintain own professional and clinical integrityin line with the NMC Code.
- To effectively engage with all team membersconcerned with patient care.
- To support the induction, supervision and teachingof junior team members, including student nurses and temporary staff.
- To be an advocate and role model within thecommunity setting.
- To be aware of budgetary constraints and effectiveresource management.
- To ensure that local and national Health and Safetyregulations are adhered to by all members of the team.






- To report and escalate any risks and follow FCsincident reporting process.
- Be aware and act upon procedures that are in placeto protect vulnerable individuals.
- To attend and engage in organisational meetings andfeedback to the team.
- To take an active part in clinical supervision andreflective practice sessions.
- To assist in service audits where indicated,supporting the continued review of effective and efficient practice.
- In the absence of the senior nurse, escalate anystaffing issues which may impact on providing safe and effective care.
