Job overview

Tri-Borough Under 5’s Pathway is a trust wide specialist team that offers therapeutic support to families during pregnancy or with infants, toddlers or children under the age of five years old.

It is part of an Early Years provision that goes from universal offer to specialist interventions. The service works in partnership with all the services involved with families who are pregnant or have an infant / young child under the age of 5.

The post holder will provide highly specialised assessment and formulation to parents / primary caregivers and their infants as well as deliver individualised evidence-based care packages where indicated including both group and individual therapeutic interventions.







They will engage in multi-disciplinary team (MDT) working with colleagues, supporting relational and safeguarding risk planning and care planning for families.

They will also promote the growing knowledge and skills regarding infant mental health across the early years system by offering consultation, advice and delivering training across the network.

This post will involve travelling between family centres and the CAMHS base in Enfield and occasional travel outside the borough to meet with other colleagues within the wider Tri-Borough.

Main duties of the job

- To be a key point of contact for families / carers allocated to you, identifying gaps in their support and enabling the system to work together to bridge those gaps.
- Where required, to provide high quality, direct support to the children and young people with complex needs and their families / carers when they need it mostly.
- To work closely with generic CAMHS, local CAMHS providers (including crisis teams),





social care and education to ensure that the system is working in a coordinated and joined up way for the young person and their family / carers and helping to reduce any blockages to accessing that support.
- Facilitate the provision and implementation of tailored behavioural and emotional support to help a child or young person and their families / carers develop skills for progress and self-management to meet their agreed goals and outcomes.
- Communicate clearly with the child, young person and their family / carer to help them understand and navigate the system.
- To work within and contribute to, detailed risk assessments for the young people; To ensure children and young people you support have the opportunity to make choices,





develop their independence skills and access a range of positive activities both in the centre and in the community.

- To represent the service at meetings, including Care Education and Treatment Reviews (CETR’s) Care Programme Approach (CPA) meetings, as well as Child in Need (CIN) meetings, Child Protection Conferences, Core Groups, Looked-After Child (LAC) Reviews, and Education, Health, and Care (EHC) Annual Reviews, as appropriate.
- To report to the service management for updating on cases at the local Admission Avoidance (Dynamic Support) Register meeting.
- To use a holistic approach based on health and social work assessments and the outcomes identified through the team around the child or in the education, health and social care plan.
- To prioritise and utilise regular one-to-one and group supervision in order to develop and reflect on own practice and share knowledge and expertise to enhance service delivery and improve outcomes for families / carers..
- To ensure high standards of case recording using the relevant information systems in order to capture all necessary inputs, outputs, and outcomes for young people and their families and carers.
- To work flexibly to suit the needs of children and young people with complex needs and their families / carers; this may include early starts, late finishes, evenings and weekends as required.
- To undertake other duties commensurate to the grade of the post.

ADDITIONAL :

- To achieve service outcomes and outputs, and personal appraisal targets, as agreed with your line manager.
- To consistently manage your own time, planning your work in advance and using appropriate systems to maximise your effectiveness.
- To undertake training and constructively take part in meetings, supervision, seminars and other events designed to improve communication and assist with the effective development of the post and post holder.
- To adhere to the : Code of Conduct of the relevant professional body, such as Health and Care Professionals Council (HCPC) or Nursing and Midwifery Council (NMC)

Person specification

Qualifications / Education and Experience

Essential criteria

- Completed a degree in psychology or in a core profession such as Occupational Therapy, Mental Health Nursing, Education, Social Work and completed (or soon to complete) a Postgrad qualification training in Infant Mental Health
- Experience of direct therapeutic work with children and young people under the age of 5 and their parents or carers presenting with social emotional difficulties.

Skills / Abilities and Knowledge

Essential criteria

- The ability to work flexibly and responsively to provide support when children, young people, families / carers need it most, which may include early morning, evenings and weekends as required.
- Some knowledge of relevant legislation, guidance, government policy and research in relation to children and young people with learning disabilities and / or autism and an ability to apply that knowledge in an operational setting.
- An awareness of the risks associated with children under the age of 5 with social, emotional and mental health needs, with the ability to use this knowledge to safeguard the children and young people in your care;

to promote and safeguard the welfare of children and young persons in your care, respond to and report any concerns about the welfare of a child promptly in line with local guidance on safeguarding children.

- Experience of delivering structured therapeutic interventions such as VIPP and Incredible Years under supervision.
- Ability to work therapeutically within and contribute to multi-agency risk assessments for children and young people with complex needs, including risks associated with behaviours of concern and support for children and young people with complex mental health needs.
- Ability to support positive, planned risk-taking that promotes developmental growth and children and young people’s rights to community access.
- Ability to deliver good outcomes for children and young people and their families / carers, working within planned interventions agreed by the professional network.
- Ability to form and maintain appropriate professional relationships and boundaries with children, young people and families, carers, colleagues and the multi professional network.
- Knowledge of child development, autism, learning disabilities, attachment, and trauma, and of personalised and coproduced care planning.
- Ability to creatively engage, communicate and consult with children and young people with learning disabilities and / or autism and their families / carers about developing and improving their service.
