Job summary

We are seeking a motivated, compassionate, and dynamic Physiotherapist with experience in elderly care and comprehensive health needs assessment to apply for this exciting opportunity to work as a Specialist Physiotherapist in the Community Ageing Well Service in Barnet.

The service is funded through Ageing-well and is focused on providing proactive holistic / comprehensive geriatric assessments.

The aim is to facilitate coordinated care for those with complex health needs and ultimately reduce unplanned hospital admissions.

The team coordinates and chairs a virtual MDT twice a week with the core team and a geriatrician from BGH / RFH and monthly with an old age psychiatrist from BEH.

Main duties of the job







Be an active member of the Community Ageing Well service, providing a clinically comprehensive Physiotherapy service and anticipatory care for adults 65+ living in the Borough of Barnet.

As part of this Ageing well-funded service you will provide proactive holistic patient centred assessment, treatment, and care to minimise unplanned care hospital admissions, optimise patient's health conditions, facilitate care closer to home, improve patient and carer experience, reduce avoidable harm (PU / Falls) and to increase the number of patients dying in their preferred place care.

Job description

Job responsibilities

The post holder will be responsible for :

Undertaking specialised uni-professional and multi-professional assessments a part of necessary to the comprehensive geriatric assessment, and re-assessments of treatment programmes using evidence-based outcome measures and in accordance with CSP Guidelines.

Working within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients / clients and maintaining professional documentation.

Being responsible for the supervision of junior staff, rehabilitation assistants, physiotherapy students,





Frailty MDT Administrators, and Frailty Support Workers.

Frailty Clinicians working within the organisation may at times be required to work with other teams, from other bases or localities.

Supporting in the operations of the team.

Person Specification

Education / Qualification

Essential

- Diploma / degree in Physiotherapy
- Current HCPC registration
- Evidence of up to date CPD portfolio
- Involvemn Appraisal Scheme
- Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Falls, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation, Palliation
- Member of CSP

Experience

Essential







- At least 2 years experience post registration as a Band 5 physiotherapist
- Experience working at a Junior level in a wide range of specialised clinical areas, providing care to frail and elderly patients with long-term conditions and / or complex health needs including outpatients, neurology, rehabilitation, orthopedics and respiratory.
- Experience in supervising, appraising and coordination of junior staff / support workers
- Experience of negotiating and liaising within a multi-disciplinary team in NHS and social services

Desirable

- Experience working with voluntary organisations
- Experience in service development projects.

Skills & Knowledge

Essential

- Skills in the assessment and treatment d,





identifying any risk or potential problem and act appropriately effective record keeping, with adults with complex conditions and multi pathologies
- Knowledge and understanding of complex medical conditions including multipathologies and falls
- Knowledge of role of Allied Health Professionals and interdisciplinary team working
- Ability to apply core physiotherapy skills across a range of clinical conditions within a client centred, goal setting.
- Knowledge of outcome measures and evidence-based practice associated with physiotherapy
- Ability to problem solve, prioritise, and demonstrate clinical reasoning
- Good communication and report writing skills, in both verbal and written format to accurately report and feedback information to colleagues and others.
- Basic IT skills Microsoft Word, Power point,





Excel
- Teaching of junior staff / students, peers in inservice training setting
- Knowledge and understanding of the role of Social Services and voluntary agencies
- Knowledge of research and audit processes
- Excellent skills in literacy and numeracy
- Knowledge and understanding of National Service Framework for Older People and Long-Term Conditions, Aging Well, other NICE related guidelines on frailty, palliation, pain management and other long term conditions
- Knowledge and understanding of NICE and CSP / COT Fallers Guidelines
