Nurse Associate - Band 4

City Community Team

Based : Five Rivers Salisbury

Full time Hours per week, permanent

A fantastic opportunity has arisen for aQualified Nurse Associate to join out City Community Team based in SouthWiltshire.

The role of the Nurse Associate will be caring for complex patientsin their home environments. The skills required for Nurse Associate includecomplex wound management, palliative care and end of life care, as well ashaving a high standard of holistic assessment and care planning skills.

that are expected to work seamlessly across multi-professional communityservices and to forge close links with other local teams, including PrimaryCare and Salisbury Foundation Trust.

Main duties of the job

The successful candidate will be able toprovide clear evidence of up to date and relevant clinical practice andeducation as well as an appetite for innovative practice.

Applicants need tohave enthusiasm and commitment to working within a community-based team.

Applicants need to have well developedcommunication skills, good IT knowledge, and evidence of basic clinical skillsand will need to be able to work under pressure.

The role is demanding butrewarding, and we are looking for an enthusiastic, motivated, and hardworkingindividuals who can work flexibly and efficiently.

The team operates variousshift patterns from until , 7 days a week.

Essential







- Level 5 Nursing Associate qualification
- NMC registration and Nursing Associate

Desirable

UK core skills Framework Mandatory training

Experience

Essential

- Some experience of working within a health and social care environment
- Ability to communicate with members of the public and health and care providers
- Courteous, respectful and helpful at all times
- Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact
- Knowledge of when to seek advice and refer to a registered care professional

Desirable

- Ability to work on own initiative
- Ability to take part in reflective practice and clinical supervision activities
- Understanding of evidence based practice
- Understanding of basic physiology,





normal vital signs, fluid balance, nutritional requirements etc.

Additional Criteria

Essential

- Access to a vehicle that can be used for business purposes and hold a full and valid driving licence.
