Job summary

The post holder will work within the Hospital and care at home(H&C;@H) team alongside the multi-disciplinary team to provide and maintain a high level of service to service users accessing NHS services.

In the absence of the clinical lead the post holder will provide clinical and managerial leadership for the H&C;@H team ensuring that safe and effective quality of care is delivered within the resources available while ensuring that the care environment is organised to meet the needs of the patient enhancing their experience and supporting staff to meet the Divisional and Trust objectives.







You will work as an autonomous practitioner assessing patients' needs and using evidenced based clinical skills alongside digital technology to care for patients in their own home.

You will also be responsible for caseload management, handover of care and service developments providing specialist input when necessary.

There will be opportunity to complete the non-medical prescribing and clinical examination modules.

Main duties of the job

- The post holder will be an autonomous practitioner, continually developing a highly specialised knowledge base and clinical assessment skills, working within agreed protocols.
- The post holder will deliver, implement, monitor, and escalate patients on the H&C;@H virtual ward and where appropriate liaising with medical colleagues in order to assess, implement and modify treatment or to facilitate timely reassessment of patients on the H&C;@H service.
- They will ensure appropriate pathways of care and communication via liaison and referral to other agencies and specialist teams via the MDT as required.
- The post Holder will ensure compliance with Professional policies,





National and local guidelines and Trust policies, procedures, and protocols ensuring they work within their scope of practice and clinical competencies.
- The post holder will also play a key role in the delivery and development of formal and informal education to members of the multi professional team.
- Will utilise advanced knowledge and skills and provide appropriate care, clinical reasoning skills and evidence-based practice to provide specialist advice and support for individuals or the multi professional team in the care of patients within the H&C;@H service.
- Work as an autonomous practitioner; without direct supervision, drawing on advanced theoretical and practical experience.

Job responsibilities

- Responsible for prioritising and planning own and the teams workload on a day-to-day basis.






- To utilise advanced knowledge and skills and provide appropriate care, clinical reasoning skills and evidence-based practise to provide specialist advice and support for individuals or the multi professional team in the care of patients within the H&C;@H service.
- Work as an autonomous practitioner; without direct supervision, drawing on advanced theoretical and practical experience.
- Negotiate, agree expectations, and set appropriate realistic and individual goals in partnership with the patient, carer and colleagues
- Ensure that assessments, programmes of care, and evaluations related to patients being accepted and onboarded onto the H&C;&H; pathways comply with the Trusts agreed clinical standards.






- Ensure provision of holistic and rapid assessment of patients referred in a timely manner to support early discharge from the acute hospital provision.
- To undertake specialised clinical assessments of patients who are at risk of clinical deterioration at home and deliver programmes of care according to clinical need within agreed protocols after escalation and discussion with medical colleagues and the wider MDT.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months.

Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

The NCA can no longer accept applications from International Nurses and Midwives who are yet to complete all competencies needed to receivea full NMC PIN to practice in the UK as we have paused our OSCE programme indefinitely.

- Previous B6 experience in speciality
- Evidence of CPD
- NMP or prepared to undertake
- Enhanced clinical skills qualification or prepared to undertake

Evidence of teaching / mentorship qualification

Knolwedge and Skills

Essential

- Evidence based clinical knowledge
- Substantial post reg experience (4 years+)
- Evidence of clinical and managerial skills at B7
- Excellent communication skills
