Job summary

This role involves coaching and motivating patients in managing their own health and wellbeing to live independently and improve their health outcomes.

The post holder will work with individual and / or groups of patients to deliver sessions on healthy eating, physical activity and health and well-being to meet personalised care plans.

The role works within their practices Multi Disciplinary Team; induction and training is given to help candidates integrate into Primary Care.

This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within primary care.







This is a full-time role covering 4 day at the Huddersfield Road Surgery and 1 day at Darton Health Centre.

Main duties of the job

Health coaching uses coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.







You will work in partnership with your clinical and non-clinical colleagues across the PCN to ensure the role delivers the best possible outcomes for our patients.

The role is varied, and may include supporting self-management education, peer support, case management and facilitating group consultations.

You will take an approach that is non-judgemental, based on strong communication and negotiation skills. You will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions.

Your role and skills will support and encourage the prevention of developing further illness, or the deterioration of existing long-term conditions.

PRIMARYDUTIES AND AREAS OF RESPONSIBILITIES

Healthand wellbeing coaches will :

Coachand motivate patients through multiple sessions to identify their needs, setgoals,





and support them to implement and achieve their personalised health andwellbeing objectives.

This will include sitting in consultation with a patientand providing them with advice, guidance, and a management plan that ispersonalised to their needs.

It could include dietetics and healthy eating,lifestyle medicine and getting active, safe activities and personal coachingand motivation.

Manageand prioritise your own caseload in accordance with the health and wellbeingneeds of the patient population.

Ensureall interventions and coaching are designed to empower patients to be activeparticipants in their own healthcare; empowering them to manage their ownhealth and wellbeing and live independently.

Supportand deliver group consultations,





in collaboration with clinical andnon-clinical colleagues, implementing new approaches to health across primarycare and supporting individuals to access peer support.

Workwith the Social Prescribing Link Workers and the broader PCN colleagues toconnect patients to community-based activities which support them to takeincreased control of their health and wellbeing.

Workwith clinical colleagues to identify and provide enhanced support to those whowould benefit the most from health coaching.

Providea combination of in person, telephone and video consultation.

PatientCoaching, Care and Support :

Providepersonalised coaching to patients which supports them to identify and meettheir individual lifestyle, health and wellbeing goals.

Deliverand facilitate group and peer sessions,





including sessions focussed on specificlifestyle medicine approaches and long-term conditions.

Tailorhealth and lifestyle advice effectively to the individual ( adaptingphysical activities for individuals with mobility issues).

Buildtrust and respect with the person, providing non-judgemental andnon-discriminatory support, respecting diversity and lifestyle choices.

Workfrom a strength-based approach focusing on a persons assets.

Increasepatient motivation to self-manage and adopt healthy behaviours.

Workwith patients with lower activation scores to understand and increase theirlevel of knowledge, skills and confidence (their Activation level) to managetheir own health and wellbeing,





and increase their ability to access andutilise community support offers.

Bean approachable professional and engaging source of information about health,wellbeing and prevention approaches.

Supportpatients in shared decision-making conversations.

Workwith the social prescribing team to ensure that individuals can access supportto address wider social needs that can impact on their health and wellbeing,such as such as debt, poor housing, being unemployed, loneliness and caringresponsibilities.

PCN :

Promotehealth and wellbeing coaching, its role in self-management, addressing healthinequalities and the wider determinants of health.

Provideeducation and specialist expertise to fellow PCN staff,





ensuring they are madeaware of health coaching and social prescribing services and support colleaguesto improve their skills and understanding of personalised care, behaviouralapproaches, and ensuring consistency in the follow up of peoples goals wherean MDT is involved.

Raiseawareness within the PCN of shared decision making and decision support toolsand supporting people in shared decision-making conversations.

Engagewith and support the new and evolving agendas and service requirements acrossthe PCN, including our work with Care Homes.

Buildrelationships with staff in the GP practices, attending relevant MDT meetings,giving information and feedback on health coaching.

SystemResponsibilities :

Developand maintain effective relationships with the VCSE sector.

Seekregular feedback about the quality of service and impact of health coaching onreferral agencies.

Beproactive in encouraging equality and inclusion.

Develop collaborative relationships and workin partnership with health, social care, and community and voluntary sectorproviders and multidisciplinary teams to holistically support patients widerhealth and wellbeing, public health, and contributing to the reduction ofhealth inequalities.

Datacapture :

Worksensitively with people, families, and carers to capture key information,enabling tracking of the impact of health coaching on their health andwellbeing, including the measures required within the PCN Contract ( PAMmeasures).

Encouragepeople, families, and carers to provide feedback.

Supportreferral agencies to provide appropriate information about the person they arereferring.

Provide appropriate feedback to referralagencies about the people they referred.

Ensurethat the relevant codes are captured and inputted into clinical systems, (asoutlined in the Network Contract DES), adhering to data protection legislationand data sharing agreements.

Professionaldevelopment :

Workwith your line manager to undertake continual personal and professionaldevelopment, taking an active part in reviewing and developing roles andresponsibilities.

Undertake Helath and Wellbeing coaching accredited training as required by the Personalised Care institue.

Adhereto organisational policies and procedures, including confidentiality,safeguarding, lone working, information governance, equality, diversity and inclusiontraining and health and safety.

Other :

Workas part of the healthcare team to seek feedback, continually improve theservice and contribute to business planning.

Contributeto the development of policies and plans relating to equality, diversity, andhealth inequalities.

Undertakeany tasks consistent with the level of the post and the scope of the role,ensuring that work is delivered in a timely and effective manner.

Person Specification

Experience

Essential

- Educated to GCSE level or equivalent.
- Demonstrable commitment to professional and personal development.
- NHSE Approved training in motivational coaching, behavioural change and goal setting.
- Knowledge of the personalised care approach.
- Understanding of the determinants of health to include social, economic, and environmental factors.
- Understanding of, and commitment to, equality, diversity and inclusion.
- Experience in health promotion.
- Excellent written and spoken skills with the ability to communicate effectively at all levels across the PCN.
- Good technical literacy with Microsoft Office packages.
- Able to work independently and manage own workload.
- Able to build strong professional relationships.
- Able to work as part of a team.
- Full UK Driving License.

Desirable

- Holds relevant NVQ Level 3 qualification or equivalent experience.
- Knowledge of public health issues in the local area.
- Knowledge and experience of training volunteers.
- Knowledge of the needs of patients with long term conditions.
- Knowledge of community development approaches.
- Experience of working directly in a community development context, adult health and social care, learning support or public health / health improvement.
- Experience of supporting people, families and carers in a related role.
- Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity.
- Experience of data collection and using tools to measure the impact of services.
- Experience of partnership / collaborative working and of building relationships across a variety of organisations including the voluntary sector.
- Knowledge of clinical systems.
