Job summary

Are you a newly qualified or experienced nurse wishing to pursue acareer in General Practice? Would you like to work in a community-based settingproviding holistic, person-centred care in a multi professional team?

TowerHamlets GP Care Group CIC in partnership with our local GP employers aredelighted to offer either a 2 year or 12m fixed term training post.

You will beworking within a General Practice setting alongside studying at City Universityof London for a Post Graduate Diploma (PG Dip) in Practice Nursing.

Alongsidethis you will be supported by our pioneering Nursing Development Open Doors clinicaltutors.







Our innovative programme has been running continuously since 2008and we are proud to say many of the Open Doors nurses have continued workingwithin Tower Hamlets and gone onto become Lead Nurses, ANPs and Educators.

Withmany years of experience our educators will guide you through your journey intoprimary care.

The University fees are fully funded.

The successful candidate will be employed by an appointed GP practice and work with the whole PracticeTeam. You will spend hours per week partly working clinically and partly having time out tostudy.

This is arranged flexibly to meet the needs of the practice and fit inwith study needs of the nurse and clinical experience needed.

Main duties of the job

- Positivelyimpacting the public health of patients and residents in Tower Hamlets viavaccinations, screening, health promotion.
- Providingcare to those with complex and long term conditions including physical andmental health problems, enabling them to stay well and stay out of hospital.
- Supportingfamilies and carers who are caring for those with mental or physical problemsand distress.






- Maintaining and improving theexcellent reputation of Tower Hamlets as a caring and learning community ofpractitioners working in partnership.

The Open Doors team provides :

Weekly visits which last around 3 hours in year 1 to assist with the development of clinical skills. These are tailored to meet the needs of the individual nurse and delivered in the workplace.

Visits are every 2 weeks in year 2

Weekly half day action learning group in year 1 and monthly in year 2, which provides further teaching and is a safe space for you to bond with colleagues

Preceptorshipsupport in your new role is built into the programme for any newly qualifiednurse.

Job description

Job responsibilities

Clinical

- 1. Provide travel advice and vaccinate appropriately;
- 2. Assess and care for patients presenting with uncomplicated wounds, including removal of sutures and clips;
- 3. Support and advise patients requesting information relating to reproductive and sexual health;

Communication

1. Communicate effectively within the MDT teams in general practice, patients and carers.

2. Anticipate barriers to communication and take action to improve communication;

3.





Act as an advocate when representing the patients and colleagues viewpoints to others.

Administration

1. Be curious about of the wider primary care system Primary Care Networks, Integrated Care Boardss, Integrated Care Systems and neighbourhoods;

2. To maintain accurate patient records including entering onto the computer using agreed Snomed codes

3. Participate in practice audit and research projects as required

General

- To maintain own annual registration with the NMC and professional indemnity and adhere to The Code : Professional standards of practice and behaviour for nurses, midwives and nursing associates
- Ensure all statutory regulations ( Health & Safety, COSHH, etc.) are implemented and adhered to






- To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice
- To maintain own continuous professional development (CPD) and ensure CPD is kept up-to-date through annual appraisal in line with the requirements of the Practice and The Code
- Attend and participate in staff and Practice meetings and in-house clinical training sessions. Attend ALGs provided by the Open Doors Tutors
- Working within limitations, being aware of own boundaries

Qualifications

Essential

- Educated to degree level obtaining First Class / 2 : 1 qualification
- Registered with the Nursing & Midwifery Council

Knowledge and Experience

Essential

- thorough and up-to-date knowledge of nursing theory and best practice at the level of a newly qualified nurse
- Knowledge of primary health care and General Practice
- Understanding of equality & diversity and how to apply it

Skills and Abilities

Essential







- Demonstrates an empathetic and caring approach to patients and relatives and ensure that patients dignity & respect is maintained at all times
