Job summary

Due to a growing population, we arelooking for a new Advanced Care Practitioner to join our team.

Applications from Advance Nurse Practitioners, Paramedics orprescribing nurses are welcome

Would consider part-time or full-time

Experience in primary or urgent care preferrable

Salary based on experience

Main duties of the job

Responsible forattending to patients who wish to be seen by a health care professional. Diagnosingand treating patients presenting with minor illness, whilst seeing and advisingpeople in respect of their continuing medical and nursing needs.

Total triageis in place for all requests for treatment. There is a requirement to see extraor emergency patients where the situation demands.







The practitioner will needto prioritise using telephone triage the needs of patients, accordingly, makingany necessary referrals for investigations in the appropriate manner.

Patientswith problems needing referral to secondary care should be discussed with theregistered GP.

Haresfield Surgeryis situated within the Worcester City locality boundary and provides generalmedical services to approximately 18,000 patients.

The Practice has sevenpartners, seven Salaried GPs, Advanced Nurse Practitioners, Registrars, GPClinical Pharmacist, dedicated Management and a strong Nursing Team, all ofwhich are supported by skilled Receptionists and Administrators.

It is ideallylocated with excellent road links to surrounding towns and villages.

Job responsibilities

Principal Duties and Responsibilities

Assess,diagnose, plan, implement and evaluate treatment / interventions and care forpatients presenting with an undifferentiated diagnosis.

Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly.

Assess,diagnose, plan, implement and evaluate interventions / treatments for patientswith complex needs and proactively identify, diagnose, and manage treatmentplans for patients at risk of developing a long-term condition (as appropriate).

Diagnoseand manage both acute and chronic conditions,





integrating both drug- andnon-drug-based treatment methods into a management plan.

Prescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice.

Workwith patients to support compliance with and adherence to prescribed treatments.

Provideinformation and advice on prescribed or over-the-counter medication onmedication regimens, side-effects, and interactions

Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare.







Supportpatients to adopt health promotion strategies that promote healthy lifestyles,and apply principles of self-care.

Supportand manage womens health needs.

Assess,identify, and refer patients presenting with mental health needs.

Implementand participate in vaccination and immunisation programmes for both adults andchildren.

Promoteand deliver evidence-based care for patients presenting with aural conditions.

Utiliseand demonstrate sensitive communication styles, to ensure patients are fullyinformed and consent to treatment.

Communicatewith and support patients who are receiving bad news.

Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background, and preferred ways of communicating.







Anticipatebarriers to communication and take action to improve communication.

Maintaineffective communication within the practice environment and with externalstakeholders.

Actas an advocate for patients and colleagues.

Ensureawareness of sources of support and guidance (, PALS) and provideinformation in an acceptable format to all patients, recognising anydifficulties and referring where appropriate.

Delivering a quality service

Recogniseand work within own competence and professional code of conduct as regulated bythe NMC or other regulatory body.

Produceaccurate, contemporaneous, and complete records of patient consultation,consistent with legislation, policies and procedures.

Prioritise,organise, and manage own workload in a manner that maintains and promotesquality.







Delivercare according to NSF, NICE guidelines, and evidence-based care.

Assesseffectiveness of care delivery through self and peer review, benchmarking, andformal evaluation.

Initiateand participate in the maintenance of quality governance systems and processesacross the organisation and its activities.

Utilisethe audit cycle as a means of evaluating the quality of the work of self andthe team, implementing improvements where required.

Inpartnership with other clinical teams, collaborate on improving the quality ofhealth care responding to local and national policies and initiatives asappropriate.

Evaluatepatients response to health care provision and the effectiveness of care.







Supportand participate in shared learning across the practice and wider organisation.

Use astructured framework ( root-cause analysis) to manage, review and identifylearning from patient complaints, clinical incidents and near-miss events.

Assessthe impact of policy implementation on care delivery.

Understandand apply legal issues that support the identification of vulnerable and abusedchildren and adults, and be aware of statutory child / vulnerable patientshealth procedures and local guidance.

Workwith practice management to ensure the whole team have skills and knowledgeregarding domestic violence, vulnerable adults, substance abuse and addictivebehaviour.

Provideguidance and support to ensure appropriate referral if required.

Person Specification

Qualifications

Essential

- Registered Nurse
- Post graduate diploma or degree (Advanced / Autonomous Practice Qualification)
- Qualified Nurse Prescriber

Desirable

- Qualified Triage Nurse
- Minor Illness Qualification

Experience

- Experience of working in a primary or urgent care ( A&E;) environment
- Experience of chronic disease management
