The Complex Care and Review team is a small but extremely effective team consisting of a Practice Manager, x Social Workers, x Social Care Assessors, x Occupational Therapist and x OTA.

This is an experienced team of highly motivated, hardworking, and skilled professionals.





We are looking for a candidate who demonstrate these skills and have a passion to drive forward positive outcomes for individuals regardless of their level of need or stage of life they are in.

Candidates will be required to work collaboratively and creative with colleagues, and to have an in-depth knowledge of the Care Act and a sound knowledge of current legislation, policy and procedures within adult services.

The team is unique in that it completes reviews taking a whole service / provider’ approach. In doing so they work closely with providers and commissioners to improve the standards of current services.

This includes setting clear outcome focused care plans, taking an innovative approach regarding effective use of current services,





and scrutinising services to ensure commissioned support is required and being used appropriately in keeping with a person-centred and strength-based approach.

Their remit also includes project work, working closely with operational teams and commissioners in the future forecasting of service-user’s needs within CW&C;, identifying gaps in services and steering new bespoke commissioning opportunities.

The complex care and review team, work across several areas of social care including Transitions, Learning Disabilities and Mental Health (Borough wide).

We are looking for dedicated and highly motivated team players to join us as Social Workers working in line with the Care Act .

Our Social Workers play a critical role in supporting people to develop creative and innovative solutions to meet the outcomes they want to achieve.

- Experience working with people with Learning Disabilities and or Mental Health needs and an understanding of the policy context surrounding these areas of practice.
- Understanding of the complex working areas of forensic case work
- Understanding of complex health and care needs

You will have :

- CQSW / CSS / Diploma / Degree in Social Work or equivalent (Essential)
- Social Work England Registration (Essential)






- Be required to undertake an enhanced DBS plus Adults Barred List (Essential)
- Statutory and policy base of Social Work (Essential)
- Appreciation of recent developments in service provision.
- Ability to communicate effectively with customers, carers and families.
- Ability to cope positively with change and an ability to promote positive changes to make a difference.
- Awareness of importance of multi-disciplinary working and an integrated approach.
- Ability to carry out assessment and support planning in line with relevant legislation.
- You are confident, energetic and enthusiastic as well as sensitive and resilient.
- Self-motivating individual with the passion to make a difference in adult social care.
- Resilient personal stress management.







This is a hybrid post which means working in one of our Council offices in locations across the borough as well as the opportunity to work from home where suitable.
