The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable.

Detailed job description and main responsibilities

Communication and engagement
• Responsible for communicating and maintaining relationships across a range of professional and clinical boundaries. Communicate
the planning and implementation of care in partnership with patients, the parent team and families/carers as appropriate.
• Maintain current, clear, comprehensive documentation to record interactions and provide continuity of care between patient and
those caring for them in line with the NMC code of conduct (NMC, 2018). Actively promote the critical care outreach team both
within and outside of the health board
• Establish effective relations and communication networks with internal and external agencies, including all Wales and national
networks.





Facilitate the sharing of best practice/ideas amongst staff across the health board and externally, including national
organisations. Provide strong professional and clinical advocacy for the patient in situations where there are challenging
relationships and conflicts of interest
Quality/service improvement
• Responsible for the development of quality and service improvement strategies, through the auditing and analysis of service data,
supporting and developing quality improvement projects. Lead role in continuous improvement and innovation, underpinned by a
culture of learning where staff maximise their potential and feel they are part of the success
• Identifies opportunities for service improvement, this may include identifying the need for change, developing cases for change,






leading innovation and implementing the change
• Demonstrates self-direction and originality in tackling and solving problems and acts autonomously in planning and implementing
tasks at a professional level. Lead on and/or support the development of projects and share learning from these processes through
the presentation of audit and research data in a variety of forums
• Responsible for supporting the delivery, management and development of critical care outreach services in line with the National
Outreach Forum’s Quality and Operational Standards (NORF, 2020) and the Guidelines for the Provision of Intensive Care Services
(FICM, 2022)
Clinical
• Deliver clinical care as an outreach practitioner at an enhanced level,





working as part of a 24-hour rota supporting the management
of the deteriorating patient. Responding to a range of calls for support from a variety of adult clinical specialties across the hospital
site, managing and delegating their workload effectively. Assess and treat the acutely deteriorating patient to prevent admission to,
or manage safe transfer to critical care in a timely manner in line with health board policy and NICE guidance (2007)
• Review patients on discharge from critical care within a 48-hour window, assisting in their rehabilitation and to provide ongoing
monitoring. Support the delivery of critical care management strategies outside of the critical care unit
• Be an active member of the medical emergency team,





supporting the management of clinical emergencies across the hospital site
• Provide safe escort and transfer of acutely/critically ill patients both inter and intra hospital where required

• Where appropriate administer and titrate medications in consultation and under direction of the medical team. Administer
emergency medications where indicated using specific patient group directives (PGD). Where a non-medical prescribing qualification
is held,





can prescribe medications in line with the practitioner’s agreed scope of practice
Management & Leadership
• Responsible for their own development of knowledge and skills and to support the development of others. Maintain and advance
their own knowledge through continuing professional development (CPD) and support other members of the team in this where
required and ensure their own statutory and mandatory training is kept up to date Prepare for and take an active part in the PADR
process in accordance with health board policy, identifying one’s own learning and development needs
• Engage with one’s own NMC revalidation to maintain professional registration and support other members of staff in this process
• Support and contribute to the recruitment, selection of staff within the team






• Contribute to ongoing learning, education and development at a health board wide level, promoting development around the acute
deterioration agenda, in partnership with invested teams and departments
• Provide management and leadership both within the team and across the hospital site through their day-to-day interactions with
staff. Carry out PADR reviews for allocated members of staff in line with health board policy
• To be responsible for supporting the learning and development of student healthcare professionals and staff members spending time
with the critical care outreach team
• Responsible for the management of the team’s roster (off duty) on a rotational basis, ensuring for fairness and equitability in






allocation of shifts, considering appropriate management of staff resources in line with health board policy
• Responsible for the management of the annual leave roster on a rotational basis, ensuring for fairness and equitability in allocation
of leave in line with health board policy
• Share responsibility for the day-to-day management of the team when on shift, negotiating workload and allocation of tasks
• Attend meetings on behalf of the team, presenting data where required and sharing any relevant information from the meetings
with the team
Finance and resources
• Responsible for supporting the management of day-to-day activities of the team with consideration to the appropriate management
of resources. Demonstrate an awareness of the financial restraints facing all clinical services, ensuring resources are managed and
utilised appropriately. Ensure that health board financial procedures are adhered to, e.g. patient's property procedure
Digital and Information
• Manage data and information across a variety of systems. Use of common Microsoft office software to analyse and present data
• Responsible in the maintenance of accurate records of care on the critical care outreach database, hosted on medicus
• Responsible for the accurate recording of episodes of care in the patient’s records in line with health board policy and the NMC code
of conduct (NMC, 2018) and contribute to handing over of shift data to oncoming shifts, highlighting concerns and issues from the
previous shift.
