Responsibilities:
- You will be part of a newly created all age adult mental health hospital discharge team
- Work collaboratively with Pennine Care colleagues and have a crucial part to plain in improving mental health services and outcomes for people leaving hospital and their carers
- Support older and working age adults who also have functional or organic mental health diagnosis to be discharged from hospital safely in timely manner
- Liaise with health, advocates and third sector colleagues
- Undertaking mental health capacity assessments, chairing best interest meetings, writing reports and attending tribunals
To be considered for the Social Worker role:
- 1-3 years experience of working within a similar social work setting
- Diploma/Degree in Social Work
- Registration with Social Work England
- Experience of working within a mental health setting
