Job description
An exciting job role has recently become available at the Liverpool City Council. They are looking for a new member for their Adult’s Hospital Discharge Team.

Requirements :
- Qualifications : You must have a degree in social work and be registered with Social Work England
- Experience : Significant frontline experience in adult services, particularly in hospital discharge settings, is essential.
- Skills : Strong interpersonal and communication skills are crucial, along with the ability to work independently and manage a caseload effectively.
- Knowledge :
Familiarity with the Care Act assessments and hospital discharge processes is important.
What we are looking for in a Social Worker :
- Social Work England registration
- Eligible to work in the UK
- Hold a full UK licence
- Post-qualified experience
- Degree level or equivalent in Social Work
