Job overview

We have an exciting opportunity for a Community Support Worker within Home First North Team.

The main purpose for this post is to provide a versatile and responsive service to ensure that opportunities for patients’ reablement are optimised within the community setting. The Reablement workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital.







The post holder will be expected to work with patients in their usual place of care this may also include Residential homes and nursing home. The post holder will be expected to be flexible in working patterns to meet the needs of the service and will be rostered to provide a 8-8, 7 day a week service.

Main duties of the job

- Working as part of an interdisciplinary team, to facilitate holistic reablement for patients.
- Work under the guidance and clinical supervision of the locality team including ward and community staff.
- To work as a part of a team to provide an 8-8, 7 -day service for the reablement of patients.
- Must be able to provide, receive and feedback routine information which requires tact and persuasive skills.






- Ability to persuade patients to co-operate in their reablement programme; tact and re- assurance required when dealing with patients.
- Begin working towards the completion of work-based Rehabilitation Support Worker and Healthcare Assistant (HCA)
- Completion of the Ageing well competencies training programme.
- Actively participate in own wider learning, including clinical supervision and peer supervision
- Work alongside clinical facilitator to achieve venepuncture competency, as required.
- Work within parameters of own job description, monitor own progress; strive to improve practices and performances as services
- Assist the patient to achieve their reablement goals using a range of specific activities, primarily on an individual basis, but also in groups, without direct supervision.





This may include activities for developing independence in daily living skills, e.g., supporting with personal care and promoting a return to normal function and activity as appropriate: support and encourage the appropriate use of aids/equipment and reinforce self-care wherever possible.

Education / Qualifications and Relevant Experience

Essential criteria

- NVQ 3 or equivalent experience in a range of subjects relevant to rehabilitation/ health and social care
- Previous experience of providing care/reablement in a health or social care setting
- An awareness of the needs and aspirations of those needing assistance

Desirable criteria

- Food hygiene certificate
- Infection control

Skills and Aptitude

Essential criteria

- Ability to work independently and alone
- Valid current UK driving licence
