Job summary

Our friendly and long-established heart failure service in Derbyshire is looking for a highly motivated nurse to join our enthusiastic and busy team in the north area (based in Chesterfield) to deliver high quality care to heart failure patients;

either Band 7 (if all criteria met) or Band 6 (as a training post with promotion to Band 7 on completion of criteria).

We are a proactive and innovative team working across community and acute settings in North Derbyshire.

This post is multi-faceted and involves working remotely (via telephone / video calling), within the community (in clinics / patient's homes) and in the Chesterfield Royal Hospital.







Our aim is to use resources efficiently and sustainably to provide equitable and timely access to high quality care, including self-management, in order to improve the healthy life expectancy for people with heart failure

Main duties of the job

The role includes managing a caseload of heart failure patients to ensure their treatment is optimised to give them the best possible quality of life.

You need to demonstrate motivation and commitment to giving high quality patient centred care and education.

Experience in delivering community-based care along with expertise in heart failure assessment, care and treatment is essential as well as in depth knowledge of heart failure medications, treatments and diagnostics.

We utilise a variety of telehealth solutions to support our work as well as delivering intravenous diuretics in the community.

We work closely with local community services, GPs, cardio-renal services in secondary care (with whom we have fortnightly multidisciplinary meetings) and our tertiary providers.







The successful applicant will be supported by the lead for the heart failure team and other heart failure specialist nurses.

There is a well-established induction programme and opportunities for professional development.

Please note :

To attain band 7 you will need to have achieved all the below :

- completed local DCHS and heart failure service induction programmes
- completed degree level modules of an approved heart failure course
- completed a V300 Independent non medical prescriber programme
- demonstrate ongoing competence and confidence in managing and leading the care in complex heart failure patients







At band 7 you will be required to :

- manage complex patients with heart failure in the community
- Be part of the service that provides "In Reach" to the acute Trust
- Help in the development of a rapid titration service (a joint venture of the acute trust and community heart failure service)
- Be part of the community Intravenous Diuretic Service
- Provide support and training to less experienced staff

Person Specification

Qualifications

Essential

Teaching and assessing in clinical practice qualification (eg ENB 998, C&G; 7307 or equivalent)

Desirable

- Non-Medical prescriber V300 (essential for band 7 post)
- HF module degree level 6 / 7 (essential for band 7 post)
- Cardiovascular clinical assessment skills course (essential for band 7 post)

Experience







Essential

- Community nursing experience with evidence of working with a heart failure service AND / OR Acute experience with evidence of giving heart failure care to patients as a major part of role
- Experience adjusting / titrating medical therapies in heart failure patients
- Evidence of autonomous working

Desirable

- Recent and relevant community or acute experience, working with heart failure patients, including assessment and alteration or treatment
- Recent leadership role

Skills

Essential

- Venepucture
- Up to date with skills-based competencies (RODP; BLS with AED; anaphylaxis)
- Record 2 lead and limb lead ECG
- Communications skills training (eg CBT, MI, Counselling, Quality conversations)






- Clinical examination skills for cardiology (evidence of some of the following assessment of oedema; ascites; chest auscultation; JVP assessment)

Desirable

- Interpret ECG Rhythms and 12 lead ECGs Interpret U&E; LFT and FBC results alongside clinical information (essential for band 7) Undertaking audit work
