Job summary

Triage and Referrals Mental Health Practitioner :

An exciting opportunity has arisen within the South Kensington Community Health Hub for a triage and assessment mental health practitioner.

The role is to screen referrals, undertake specialist assessments and provide brief follow up or facilitate an alternative service provision for people referred to the team.

This is a front-line post which involves face-to-face work with services users' and their carers'. The post holders will need to be able to make clear and autonomous decisions based on the complexity of the presentations due to the persons health and social care needs.

Main duties of the job

- To provide comprehensive, culturally appropriate, evidence-based assessments for adults with primary mental health need.
- To establish eligibility for health and social care services for adults with mental health needs in line with national and local policy;

the Care Act 2014 and Section 117; and any other relevant framework

To undertake continuous risk assessment and risk management in partnership with the service-user; their relatives and other professionals.

Recording and documenting using correct tools and systems.

- To co-ordinate a multidisciplinary response to crisis situations.
- To provide direct therapeutic care to users in their own home or other community settings, using evidence-based nursing skills and psychosocial interventions.






- To co-ordinate and manage all aspects of care and support, including overseeing work carried out by statutory and non-statutory agencies.

To offer supervision, training, and guidance to external agencies as required

- To assess, plan, implement and evaluate the care provided to users, in collaboration with users, carers, GP's and other members of the multidisciplinary team.
- To identify carers and to support with referring for statutory Carers Assessments and Care Plans.
- To screen for Vocational Needs and refer on for full Vocational Needs Assessment where appropriate.
- To work in partnership with other agencies ( Housing, Benefits, Social Services, etc)

Main duties of the job

The successful applicant will take the lead of a caseload of patients and be at the forefront of their care. You will also be expected to work in collaboration with the Operational Team Manager, Clinical Leads / Senior Practitioners and to have close working relationships with the Inpatient Services and Home Treatment Team.

As a case manager you will assess health and social care needs and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty.

You will proactively undertake and ensure rigorous risk assessment & risk management strategies are applied, helping to identify and mitigate risks as well as encourage individual growth and development for your patients through informed positive risk taking

You will apply specialist clinical knowledge and skills to carry out evidence-based interventions, which may be individual or in groups, which are underpinned by relevant theoretical approaches and in line with the Trusts Research and Development Strategy;

to lead and supervise junior staff in this practice as required.

Requirements of the role

Current Professional Registration as a Registered Mental Health Nurse

Evidence of post registration training and CPD

Experience of working with service users presenting with a wide range of complex mental health conditions.

Experience of working in a multi-disciplinary team

Supervision of students and junior staff.

Audit and research; Teaching; Working with people who use substances

Skills and knowledge

Excellent verbal and written communication skills, including report writing and presentations / IT skills

Ability to assess, plan, implement and evaluate a comprehensive care package for the identified client group, using a wide range of resources.

Able to develop a comprehensive programme of intervention and support for the identified client group and their families

Essential

- Current Professional Qualification in mental health nursing
- Registration must be live with professional body NMC
- Evidence of post registration training and CPD.

Desirable

Training in Psychosocial Interventions or Thorn Family Therapy / Work training; Other relevant training, substance misuse

Experience

Essential

- Significant years post registration experience
- Experience of working in multidisciplinary mental health team with clients with acute / severe and enduring mental health problems

Desirable

- Work with clients with acute / severe and enduring mental health problems who are homeless / rootless
- Work with people experiencing mental health crisis.
- Supervision of students and junior staff
- Audit and research;
