Areyou an experienced Lead Nurse Advanced Nurse Practitioner (ANP) with GeneralPractice and management experience ?

Weare recruiting for a Lead Advanced Nurse Practitioner (ANP) with GeneralPractice, management experience and good knowledge of clinical governance totake on the clinical lead responsibility for the Junction Health Centre (JHC)nursing team.

The JHC is a high QOF achieving and Good CQC rated servicelocated in Clapham Junction.

Thisuniquerole presents an excellent opportunity for an experienced ANP. Aspart of The Junction Health centre, We can provide you with the support and thetools for the role and offer you the chance to develop and diversify yourcareer.

As the Lead ANP,





you will be responsible for supporting the ManagementTeam with clinical effectiveness activities, clinical governance, CQC readinessand service improvements.

TheLead ANP will be given 4-6 hours of weekly protected management time for theseduties.

Thevacancy is a permanent contract with the option to work between 24hrs - p / w. Thiscomes with a FTE salary of £63,000 per annum.

Theworking pattern is weekdays with the option to work on weekends. Our service isopen 8am-8pm Monday-Sunday.

Main duties of the job

Responsibilities

The following are the core responsibilities of the Lead Advanced Nurse Practitioner :

- Organise the provision of nursing services for the practice to include the delegation of duties to other practice nursing staff
- Provide treatment to patients in participation with doctors or independently to agreed protocols
- Providehealth screenings to the practice patients (within agreed protocols) with referral to doctors
- Develop,





implement and embed health promotion and wellbeing programmes
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
- Set standards for patient care based on best evidenced based clinical protocols)
- Develop advanced clinical skills - x-ray interpretation, suturing, plastering
- To develop and maintain competencies associated with the role, ensuring yourself and more junior staff are working within own limitations
- Work at all times within the NMC Code of professional practice






- Personally responsible to maintain NMC registration and professional development in line with the expectations
- Have a high level of clinical expertise and leadership skills to give support and supervision to the ANPs

SERVICES APPROPRIATE FOR - GP Practices

JOB SUMMARY

The purpose of the job is to translate Practice Plus Groups clinical strategies and models of care into the delivery of care for patients being treated at the Centre.

To provide clinical and professional leadership to all nursing staff including the Health Care Assistant and to work as part of the multidisciplinary team to ensure nursing services are delivered effectively to the patients.

To work in partnership with other health care professionals and Primary Care Network practices ensuring the clinical team works cohesively and effectively to deliver a high standard of patient care.

This will involve leading by example,





facilitating professional and clinical development, ensuring all competency and training requirements of staff are met to ensure the safe and effective delivery of care.

Practice Plus Group services are registered with the Care Quality Commission (CQC) and as such the Lead Nurse will play an active part to ensure the readiness of the service for a CQC inspection.

Principal Duties and Responsibilities

Managerial

- To have responsibility for the delivery of Clinical Patient Services to achieve local and national target. Actively monitor patient satisfaction and the monitoring of patient health outcomes
- To have responsibility in ensuring relevant clinical Key Performance Indicators (KPIs) and Quality Outcomes Framework targets are met






- To have responsibility in ensuring relevant CQC outcome evidence data is collected and coordinated on a monthly basis
- Work with Lead GP to host and minute monthly clinical meetings and educational events
- Attend and contribute to the monthly Quality Assurance meeting. This meeting will provide oversight on regulatory and contractual compliance
- Responsible for the nursing team rota
- Support staff with supervision and development
- Undertake clinical staff appraisals
- Manage underperforming staff members
- Assess staff on core nursing competencies

Assist in the recruitment and induction of new staff to the unit

On call duties / responsibilities involved

- Infection Prevention and Control lead
- Deputy Safeguarding lead
- Audit schedule lead







Support the management team with planning and implementation of new services and skill mix

Participate in the roll out of new initiatives and enhance services

Support and participate in shared learning within the practice

Clinical Professional Responsibilities

- To work as an autonomously practitioner making individual decisions using NMC / ACP competencies framework and NICE guidelines
- Ensure clinical protocols are adhered to at all times

Develop, implement and embed health promotion and well-being programmes

- Set standards for patient care based on best evidenced based practice ( clinical protocols).
- Develop / maintain advanced clinical skills. requesting and interpretation of basic blood results, clinical admin, referral management etc.
- Undertake peer assessment of the core nursing competencies
- To develop and maintain competencies associated with the role, ensuring yourself and direct reports are working within own limitations / scope of practice
- Work at all times within the NMC Code of professional practice
- Personally responsible to maintain NMC registration and professional development in line with the expectations of the role
- Provide, organise and implement clinical supervision in accordance with Company policy.
- Participate in the review and development of Patient Group Directions for non-prescribing staff

Communication

Demonstrate the ability to be able to motivate and influence others through advanced communication skills.

Develop effective relationships with key stakeholders in the business, including the senior and middle management team, employees, consultants, patients, the local community, health / local authorities, local authorities, local healthcare facilities, general practitioners and suppliers.

Be actively involved in Management Forums and manage own team meetings and inter-team meetings to ensure flexible working and service provisions.

Provide and circulate minutes and all relating documents / presentations formally and informally.

Ensure that identified lines of communication are maintained with patients, colleagues and external agencies to demonstrate politeness and courtesy and sensitivity promoting the corporate image of Practice Plus Group.

Responsibility to ensure that all clinical and non clinical staff members are aware of local and general Health & Safety matters, Clinical Governance and other on-going issues, which arise including patient safety issues.

Clinical Governance

- To lead the systematic monitoring, review and evaluation of nursing practice through clinical audit within the centre.
- Work within Practice Plus Groups Clinical Governance framework.
- Report complaints, incidents and adverse untoward incidents to the Service Manager and monitor via the datix system.
- Investigate incidents using root cause analysis methodology and NPSA / Practice Plus Group tool kit.

Lead in the investigation of verbal and written complaints according to Company Policy.

Be conversant and comply with Practice Plus Groups Health and Safety Policy

Quality

- Ensure that clinical and medical audit programmes are implemented and action plans developed for deficit management.
- Ensure that the centre complies with all statutory requirements from the Care Quality Commission.
- Monitor standards of care ensuring it is delivered within robust clinical protocols in accordance with local and National guidance such as the National Services Frameworks, NICE and the DoH Best Evidence Practice recommendations.
- Responsible for the monitoring of patient satisfaction levels.
- Lead on quality initiatives and accreditation exercises as required.
- Ensure that standards of Health and Safety and Infection Prevention and Control are set, maintained and audited in accordance with company Policy and local agreements.

Leadership

- Act as a positive role model to staff and patients.
- Promote clinical excellence and best evidence based practice reflecting Practice Plus Groups nursing philosophy.

Maintain own personal and professional development plan, in line with Practice Plus Groups company and service objectives.

Support and drive your nursing teams development plans, CPD and skills advancement in order to maintain and improve contractual service coverage.

Represent Practice Plus Group within remit of role to external relevant bodies, organisations, institutions and individuals.

- Promote the centre in the local community by joining and actively contributing to local community initiatives.
- Maintains a visible presence in the centre being accessible to patients, staff and medical staff as needed.
- Participate fully in an annual appraisal process for the nursing team. Agree personal development plans that will ensure that the objectives of the individual, the team and the Centre are achieved.
- Participate and lead where asked, staff reviews, disciplinary meetings and improvement plans for the nursing team.
- Ensure PPG employee record keeping systems are utilised and kept up to date for your team members including HR databases, holiday records, recruitment & leavers processes, and training systems

Education and Training

- Motivate and co-ordinate the activities of the clinical team creating a positive working and learning environment.
- To have a proactive approach to self-development, working within the NMC Code of Conduct.
- Provide and co-ordinate teaching / training to support the development of other staff members.
- Be involved in health care initiatives and health promotion within the centre and local community.
- Lead in the systematic review evaluation and monitoring, of nursing practice through clinical audit. Providing feedback results to senior managers and all staff members to improve patient care within the unit.
- Apply up to date research-based findings to nursing practice, ensuring best evidence based practice.
- Show commitment to life long learning and a learning environment through personal example and dissemination of skills, attitude and knowledge to others.
- Work at all times within the NMC Code of professional practice.
- Maintain NMC registration and professional development in line with the expectations of the role.
- Comply with the Company Appraisal system and attend an appraisal every year and agree a personal development plan that will ensure that the objectives of the individual, the team and the Centre are achieved.

Ensure updated mandatory training requirements are met and equipment training records are maintained.

Qualifications

Essential

- Qualifications
- 1st level Registered Nurse with current registration with the UK Nursing Midwifery Council (NMC).
- Post registration qualification in
- First contact care
- Chronic Disease management
- Master degree in a health related subject, or already working towards ANP Masters
- Independent Nurse Prescriber
- Evidence of Continuous Professional development (CPD).
- Post registration qualification in autonomous practice or equivalent.
- Mental Health Experience
- Research / audit qualification undertaken at Level 3
- Teaching and assessing qualification.

Desirable

Experience

Essential

- Experienced for 2yrs+ at ANP level in an :
- Acute setting.
- Primary Care
- or another related area
- Experienced in leading multidisciplinary teams.

- Evidence of team leadership and supervision of junior staff.
- Evidence of teaching and assessing others.
- Experience of working within a multi-disciplinary team.
