If so, you should apply for our Senior Clinical Practitioner role in the new Specialist Older Adults Mental Health (SOAMHS) Service.

Exciting opportunities have arisen following a review of provision of mental health services for older people at West London NHS Trust.

We are in the fortunate position of transforming and expanding our service to meet the needs of older people with mild to severe mental health needs.







We are looking for dynamic innovative staff to be part of a team that will work therapeutically to support older patients in their mental health recovery in the community.

We are recruiting one Band 7 Senior Clinical Practitioner in Ealing. This post is based at Ealing Borough. Assessments and intervention are delivered at patients home therefore flexibility is important.

Ealing has an excellent public transport links therefore having a car or driving is not essential. Most of our clinicians use the bus on visits.

All expenses will be refunded in a monthly basis.

Main duties of the job

The post is an ideal opportunity for an experienced Nurse to build upon and advance their leadership and managerial skills.

The focus of the role is on providing clinical leadership to the Specialist Older Adults Mental Health (SOAMHS) service and in deputise for the team Manager with the day to day running of this service.







The focus of the service will be to ensure that patients receive assessment and therapeutic intervention to maximise their independence, safety and wellbeing.

The interventions will include education, individual work and group work and will involve working closely with patients, carers, statutory and voluntary agencies, within a multi-disciplinary team.

They will hold their own complex caseload of patients.

The post holder will be based in Ealing Community Team and they will be undertaking home visits, meetings at the team base and other community sites.

Qualifications

Essential

- Registered Practitioner ( nursing ) appropriate to the job role






- Degree / diploma level qualification supplemented by specialist qualification and / or experience.

Desirable

- Courses to masters level equivalent relevant to the care group
- Evidence of post registration training in relevant service area Completion of student Supervisor Course / equivalent / Practice Educators /

Experience

Essential

- Extensive post qualifying experience in relevant service area
- Experience of supervising staff and Students
- Experience of managing clinical risks with complex cases.
- Evidence of post registration training in relevant service area Completion of student Supervisor Course / equivalent / Practice Educators /

Desirable

- Experience working with patients and carer groups to develop innovative practice.






- Lived experience of mental health issues / caring for Older person with mental Health difficulties / dementia complex mental health needs / dementia

Knowledge

Essential

- Comprehensive knowledge of interventions specific to service area.
- Awareness of Trust policies and service specification.
- Knowledge safeguarding both children and adults and patient safety.
- To understand key performance indicators and how they relate to team.

Desirable

- Awareness of regional and national policies and procedures in relation to service area.
- Awareness of research and development in service areas.
- NICE guidance and governance procedures in relation to patient safety.

Skills

Essential

- Demonstrate excellent verbal and written communication skills with staff, patients, families,





carers and professionals
- Able to manage a caseload of people with complex mental and physical health needs including dementia
- Able to establish links with other services and professionals.
