Job summary

We are thrilled to announce an exciting opportunity for Clinical Pharmacists to join our Primary Care Network (PCN) team.

SAPA5 is a North Sheffield PCN encompassing six practices within the S5 postcode area. We are expanding our team of Clinical Pharmacists to build on the substantial impact the role has already made within our network.

Successful candidates will work across one or two of our member practices and collaborate closely with both the individual practices and the wider pharmacy team.

This position is offered as a permanent contract

Main duties of the job

The post holder will work within practices to support the team with medication and prescription queries,





conduct structured medication reviews, and assist in quality improvement initiatives to ensure cost-effective, safe, and environmentally sustainable medication management for the PCN.

Responsibilities include supporting the repeat prescription system, handling acute prescription requests, and managing medicines reconciliation during care transitions.

The post holder will also contribute to systems for safer prescribing and provide expert clinical medicines advice, addressing both public and social care needs of patients within the GP practices.

The SAPA5 Primary Care Network (PCN) consists of six General Practitioner (GP) practices that collaborate closely in the northern region of Sheffield.

The PCN serves a population of 38,000 patients, many of whom face significant medical and social complexities. The PCN is dedicated to making a substantial impact on reducing health inequalities and supporting the population in leading healthier and more fulfilling PCN has forged strong working relationships between the practices and is looking to work in a more integrated way with community partners.

Job responsibilities

Management of medicines at discharge from hospital

Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need on discharge.

Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients ( those with medicine compliance aids or those in care homes).

Patient facing Clinical Medication Reviews

Undertake structured medication reviews with patients and making adjustments to long and short term medication, and supporting the rest of the clinical team to make improvements to prescribing.

Management of common / minor / self limiting ailments

Managing caseload of patients with common / minor / self-limiting ailments while working within a scope of practice and limits of competence.

Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate

Medicine information to practice staff and patients

Answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams ( community pharmacy) and patients with queries about medicines.

Suggesting and recommending solutions.

Providing follow up for patients to monitor the effect of any changes

Risk stratification







Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches.

This might include risks that are patient related, medicine related, or both.

Service development

Contribute pharmaceutical advice for the development and implementation of new services that have medicinal components ( advice on treatment pathways and patient information leaflets).

Information management

Analyse, interpret and present medicines data to highlight issues and risks to support decision making.

Medicines quality improvement

Undertake clinical audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team

The post-holder is required to travel independently between practice sites (where applicable), and to attend meetings etc.

Experience

Essential

Has completed the Pharmacist Foundation programme OR a minimum of 2 years experience as a pharmacist within the NHS or community Pharmacy.

Desirable

- Has experience working in community pharmacy.
- Has experience working in a general practice or primary care setting.

Qualifications

Essential

Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.

Desirable







- Completed or working towards independent prescribing qualification.
- Completed or working towards completing the Primary Care Pharmacy Education Pathway.
