We are seeking an enthusiastic individual to Lead the Transition service for children with complex needs moving into adult services.

The applicant will have previous hospital nursing experience,





working with children and young people, experience working in a transitional role for children moving into adult services.

Please note we are unable to support applicants who require OSCE, as the Trust has a separate and limited application process

Main duties of the job

- To be hospital based for the majority of the time by providing direct specialist care, education and support for patients with transitional care and acting as a specialist resource when liaising with and facilitating the best use of all available services
- To provide care for transitional care patients, in accordance with national and Trust policies and Frameworks.
- To undertake a needs-based assessment of the current facilities, services available to patients and their families during transition and to continue to develop the service to assist patients during their transition phase
- Where possible to support young people 16 years and above when placed onto an adult ward within the Trust;
- Provide independent and unsupervised support to patients,





families when preparing the patient and their care group through the transition period
- To be flexible in the approach to the clinical area and provide clinical support as required
- In conjunction with the ward managers and specialist nurses / matron provide a suitable and safe environment for the physical and psychological wellbeing of staff, patients, and their relatives / carers.
- Be responsible for keeping up to date with the transition agenda with NHSE.
- Act as an expert, clinical role model and leader in transitional care for all staff both locally and within the wider NHS community

Job responsibilities

The post holder will lead on all components of the transitional service. The focus of this role will be to empower, educate and inform patients and their families through the transitional phase in order that they may continue their care in an adult environment.







The role promotes the Trusts values and high-performance standards. To provide the effective transition from Children and Young Peoples Services into Adult Services

To provide a clinical-led transitional service as a specialised coordinator working with a specified group of older children / young adults with complex musculoskeletal, spinal and oncology conditions and any additional complex needs.

To have considerable experience and expertise in the care and management of patients during the transition from children services to adult services, and be able to advise and support other members of the hospital and community teams

To facilitate high quality, individualised care to patients and relatives / carers without supervision in collaboration with the multi-professional team.

Provide specialist care, support,





and advice to all patients in transitional care and their families.

To work as an autonomous practitioner, take responsibility for the ongoing clinical management of this cohort of patients in the clinic and ward / department settings and into the community through liaison with their local health care providers

To act as an advocate for those families giving expert support and advice based on best practice and close communication with other members of the multi-professional team.

Qualifications / Training

Essential

Registered Children Nurse Qualification

Experience

Essential

- Previous hospital nursing post, working with children and young people
- Experience working in a transitional role for children moving into adult services
- up to date training in Safeguarding children and young people

Desirable

- Leadership skills
- Experience in delivering Training.
- Current Transition training

Skills and Competencies

Essential

- Able to work on own initiative and under pressure
- Able to work within a multidisciplinary team
- Good communication skills. Written, oral and electronic
- Ability to drive change in practices
- Ability to teach others

Desirable

Competence in using Excel & Powerpoint or will to learn







Knowledge (including specialist or technical knowledge required)

Essential

Safeguarding training to level 3 Knowledge of safeguarding - signposting and referrals
