About the role
PLEASE NOTE ; we are looking for a prescribing Pharmacist ONLY The post holderwill provide primary support to general practice staff with regards toprescription and medication queries.
They will help support the repeatprescriptions system, deal with acute prescription requests, medicinesreconciliation on transfer of care and systems for safer prescribing,
providingexpertise in clinical medicines advice while addressing both public and socialcare needs of patient in the GP practice (s).
Main duties of the job
Patientfacing long-term condition clinics See (where appropriate) patients with singleor multiple medical problems where medicine optimisation is required.
Deliveringpharmacy related aspects of the Network DES / LES such as conducting structuredmedication reviews
Reviewthe on-going need for each medicine, a review of monitoring needs and anopportunity to support patients with their medicines taking ensuring they getthe best use of their medicines.
Make appropriaterecommendations to Senior Pharmacists or GPs for medicine improvement.
Patientfacing clinical medication review. Undertake clinical medication reviews withpatients and produce recommendations for senior clinical pharmacist, nursesand / or GP on prescribing and monitoring.
Signpostto community pharmacy and refer to GPs or other healthcare professionals whereappropriate.
Telephonemedicines support.
Answerrelevant medicine-related enquiries from GPs, other practice staff,
otherhealthcare teams and patients with queries aboutmedicines.
Suggest and recommend solutions.
Managementof medicines at discharge from hospital.
Assistwith the development of a repeat prescribing process
Assist inmanaging the repeat prescribing reauthorisation process by reviewing patientrequests for repeat prescriptions and reviewing medicines reaching review datesand flagging up those needing a review.
Ensure patients have appropriatemonitoring tests in place when required

The GP surgery is the 1st to be become a limited company and has a growing list size of 15,500+. The practice has 3 Directors and 4 partners, management team, Lead nurse, trainee nursing associate, HCA and phlebotomist.
The admin team is 13 strong.

Job responsibilities
The post holderacts within their professional boundaries.
The post holderwill work as part of a multi-disciplinary team in a patient-facing role. Thepost holder will take responsibility for areas of chronic disease managementwithin the practice and undertake clinical medication reviews to proactivelymanage patients with complex polypharmacy, especially for older people, peoplein residential care homes and those with multiple co-morbidities.
The post holderwill provide primary support to general practice staff with regards toprescription and medication queries.
They will help support the repeatprescriptions system, deal with acute prescription requests, medicinesreconciliation on transfer of care and systems for safer prescribing,
providingexpertise in clinical medicines advice while addressing both public and socialcare needs of patient in the GP practice (s).
The post holderwill provide clinical leadership on medicines optimisation and qualityimprovement and manage some aspects of the quality and outcomes framework andenhanced services.
The post holderwill ensure that the practice integrates with community and hospital pharmacyto help utilise skill mix, improve patient outcomes, ensure better access tohealthcare and help manage workload.
The role is pivotal to improving thequality of care and operational efficiencies so requires motivation and passionto deliver excellent service within general practice.

Providefollow up for patients to monitor the effect of any changes.
Identifymedications associated with harm or unplanned hospital admissions throughaudits or reviewing discharge summaries, and be able to proactively optimisetheir therapy and prevent harm
Recommendchanges to the prescribing of high risk medications,
especially to high riskpatient groups
Managementof medicines at discharge from hospital.
Reconcilemedicines following discharge from hospitals, intermediate care and into carehomes, including identifying and rectifying unexplained changes and workingwith patients and community pharmacists to ensure patients receive themedicines they need post discharge.
Ensure continuity of medicines supply postdischarge
Signposting- ensure patients are referred to the appropriate healthcare professional forthe appropriate level of care within an appropriate period of time.
Assistwith the development of a repeat prescribing process at the practice
Assistin managing the repeat prescribing reauthorisation process by reviewing patientrequests for repeat prescriptions and reviewing medicines reaching review datesand flagging up those needing a review.
Ensure patients have appropriatemonitoring tests in place when required.
Identifycohorts of patients at high risk of harm from medicines through pre-preparedpractice computer searches. This might include risks that are patient related,medicine related, or both.
Servicedevelopment : contribute pharmaceutical advice for the development andimplementation of new services that have medicinal components.
Informationmanagement : Analyse, interpret and present medicines data to highlight issuesand risks to support decision- making.
Medicinesquality improvement : Undertake clinical audits of prescribing as directed bythe GPs, practice manager or Senior Clinical Pharmacist and feedback resultsand implement changes in conjunction with the practice team.
Medicinessafety : Implement changes to medicines that result from MHRA alerts, productwithdrawal and other local and national guidance.
Implementationof local and national guidelines and the local formulary.
Monitorpractice prescribing against the local health economies. Make recommendationsto GPs for medicines that should be prescribed by hospital doctors (red drugs)or subject to shared care.
Assistthe practices with keeping them up to date on how to access the localformulary. Auditing practices compliance against NICE technology assessmentguidance.
Provide newsletters or bulletins on important prescribing messages.
Educationand Training Provide education and training to primary healthcare team ontherapeutics and medicines optimisation.
CareQuality Commission (CQC) work with the general practice team to ensure thepractice is compliant with CQC standards where medicines are involved.
Provideknowledge on all public health programmes available where applicable.
Recognisesthe roles of other health and social care colleagues and develops and maintainseffective working relationships within them.
Liaises effectively with otherallied healthcare professional stakeholders as needed for the collectivebenefit of patients.

Skills / Abilities

Essential
- Masters degree in pharmacy (MPharm) (or equivalent)
- Has completed 18 month CPPE pathway (or equivalent)
