Job summary

We currently have a vacancy forenthusiastic and flexible Band 6 senior community nurse to join our communitynursing teams in the borough of Kingston.

Rated OUTSTANDING for CARE by the CQC you will be supporting the team leader and working with a team of highlyskilled and compassionate nurses nurse associates and health care assistants.

You will work closely with the teamlead to assist with managing a caseload of patients, attached to one or two GPsurgeries including case management of patients identified as being at risk ofhospital admissions.

You will act as a role model providing high quality careand skilled assessment of clients needs.





You will also support team members developing their skills and providing feedback through 1-1 meetings andappraisal.

You will need to have 2 years experience at Band 5 in a community setting and must be a Practice Assessor.You will have experience of managing patients with a range of long-termconditions and multi co morbidities.

Physical assessment and prescribingqualification are desirable.

You will work collaboratively with thewider multi disciplinary team other health and social care practitioners toprovide the best care and quality outcomes for patients and service users.

There will be opportunities for further development within the role.

You will need to be a car driver andhave access to a vehicle. The position is full time worked over various shiftsincluding weekends.

Part time and job share will be considered.

Main duties of the job

The post holder will work as part of a Health and Social CareTeam, providingskilled nursing care for patientsin the community and within the practice environment.

In conjunction with the District Nurse TeamLeader will be responsible for assessing,





planning, implementing and evaluatingprogrammes of care for service users and their carers.

The post holder will also assist with the leadershipand management of the nursing team and the organisation of nursing work asrequired.

Job responsibilities







- To Provide District Nursing and Community Care Services within the context of our primary care networks across the borough delivering integrated person centred responsive coordinated and high quality care services.
- To be responsible for the case load management in the absence of the District Nurse by planning providing and reviewing nursing care in response to the assessment of immediate long term needs of patients and their families in their own homes and other places deemed necessary.
- To assess and plan in conjunction with the District Nurse the holistic health and social care needs of service users and carers through the Nursing Assessment Process accessing care as appropriate to the individual assessed needs coordinating agreeing and implementing a care plan.

- To act as the Care Manager for an agreed number of service users with assessed nursing needs or who meet the Continuing Care Criteria with responsibility for monitoring reviewing the care plan and evaluating services.

- To continuously develop expertise in risk assessment or management and adult protection ensuring the use of these skills in practice at all times adhering to Your Healthcare policies alerting the District Nurse in their absence the Team Manager and other Team members when exceptional risks are identified.
- To be accountable as a professional for actions in relation to own and work delegated to others acting at all times within the NMC code of professional conduct.






- To be aware of the multiple needs of service users and carers acquiring knowledge of services available to fulfil these needs and teaching these methods of care to carers where appropriate.
- To ensure the continuity of care by effective liaison with colleagues in the Integrated Long Term or intermediate short term Care teams members of the Primary Health Care team and the hospital setting.
- To participate in and develop health promotion activities for service users and families in line with Your Health cares and local community priorities.
- To contribute to service development.
- To participate in Clinical Governance activities such as clinical audit and research.
- To comply with current Evidence Based Guidelines Your Healthcare and relevant Royal Borough of Kingston policies and procedures.






- Undertake performance review and appraisals in accordance with Your Healthcares policy and procedure.

Communications and Working Relationships

- To provide leadership and direction to the nursing team in the absence of the District Nurse.
- To allocate work and objectively assess the performance of team members against clear agreed criteria.
- Maintains up to date and accurate records ensuring confidentiality is maintained in accordance with Your Healthcare RBK NMC guidelines and the Data Protection Act.
- To participate fully in the use of information technology collecting and entering statistical data relevant to service area.

Support use of remote devices and mobile working within parameters of community nursing care.







- To participate in team meetings and facilitate or attend organisational meetings as required.
- To participate in and contribute to the development of the Integrated Care Team with other professionals in Primary and Social Care sectors.
- To facilitate a learning environment for pre registration nursing students and students from other health and social care settings completing reports and assessment documentation as required.
- In partnership with the District Nurse provide leadership mentorship and participate in the teaching and clinical supervision and training for Health Social Care and other staff and participates in the induction of new team members.
- Maintains professional competence and personal development by participation in mandatory and essential training which are reflected in the personal development plan.

Decision Making

- To set caseload priorities in conjunction with other members of the Integrated Health and Social Care Team that promotes peoples equality diversity and rights.
- To ensure the efficient and effective use of all resources to meet the assessed needs of service users and their carers.
- Dexterity in fine motor skills. Examples may include Intravenous procedures Syringe pump Male or Female and supra pubic catheterisations Topical Negative Pressure TNP and injectable implants.

Physical Demands

- Patient movement with use of mechanical aides manoeuvre patients
- Stand or walking for the majority of shift.

- Concentration required when checking documents / patient notes works to schedule of visits.
- Emotional Demands.
- Communicating with distressed anxious worried patients or relatives.

Person Specification

Qualifications

Essential

- Registered General Nurse
- Relevant post registration study at diploma degree level
- ENB 998 or equivalent Mentor or Practice Assessor course
- Evidence of continuing professional development

Desirable

- District Nurse Specialist Practitioner qualification or willing to work towards
- Independent Prescriber or willing to work towards non medical Prescribing
- Relevant courses in Long Term Conditions Management
- Clinical Decision Making qualification or will work towards

Experience

Essential

- Minimum 2 years community experience with demonstrable clinical expertise at band 5 community Nursing level
- Evidence of sound decision making including clinical decisions
- Experience of leading a team to achieve results
- Working within a multi disciplinary team
- Management of change within a complex environment
- Experience of teaching, facilitating and mentoring nursing students
- Experience of participating in investigations and managing of complaints
- Evidence of participating in clinical audits in the workplace
- Knowledge of Safeguarding of Vulnerable Adults

- Ability to utilise research in clinical practice
