Job summary
This is a Band 6 post however we would consider a Band 5 / 6 developmental post for the right candidate.
The BSW Specialist Community Perinatal Mental Health Service is looking for Care Co-ordinators to work within our team covering Wiltshire (North, South or Mid).
The team is multidisciplinary, providing support for women who experience serious mental health problems or present with a high risk of relapse during pregnancy and the postnatal period.
We offer assessment and treatment aimed at treating mental ill health and enhancing the mother infant relationship, where mental health needs have impacted upon this.
We provide multi agency training in perinatal mental health to other professionals and agencies working with women in the perinatal period in addition to offering consultation and advice.
Currently, the team is able to work alongside women and their families from pregnancy until their infants first birthday.
The Specialist Community Perinatal Team works across BaNES, Swindon and Wiltshire. We have a hub and spoke model - the hub is in Chippenham, Wiltshire.
The spokes are in Paulton (BaNES), Swindon and Salisbury.
This post is suitable for experienced and professionally registered Nurses, Occupational Therapists or Social Workers. You must have experience of working with women in the perinatal period or a good understanding of the specific mental health issues that can affect these women.
Main duties of the job
Perinatal Mental Health is a rapidly expanding area with significant investment being made. This means that there will be high quality training and potential opportunities for career development for the successful candidates.
You will be required to provide assessment and care co-ordination for an allocated caseload of service users with perinatal mental health needs.
This will include key tasks of undertaking high quality assessments, developing care plans, delivering and reviewing therapeutic interventions.
You will act as a supervisor and role model to others. You will also be required to act as a mentor to students, cover duty on a rotational basis and provide training to teams both within and outside of AWP.
Applicants will be expected to demonstrate a high level of decision making skills and to work independently. They must also have a good understanding of safeguarding.
Access to your own transport for work use is essential as public transport is unreliable in parts of the BSW area.

1. To undertake the full range of activities required to deliver ongoing comprehensive Mental Healthassessment for service users with severe and enduring mental health needs living in the community andin a range of settings.
This will include : a. The use of standardised assessment tools, the ClusterAllocation Support Tool (CAST), KGV b. Recovery Star c.
History, strengths and aspirations d. Mentalstate e.
Impact of culture and diversity f. Functional needs g. The needs of family and carer h.
Evaluationof risk i. Physical health j. Complicating factors k. The interventions and treatments required to enablepositive change.
l. Social care needs m. Safeguarding and public protection n. Capacity under the MentalCapacity Act2. To be responsible for developing delivering and reviewing comprehensive and complex treatmentprogrammes using appropriate frameworks in line with evidence based practice, including strategies tomanage risk for service users with complex needs and carers, bringing in other resources as To act as care coordinator for service users with the most complex needs, also providing definedinterventions to individuals on other workers To plan,
deliver and evaluate defined, specialist therapeutic interventions as indicated in theassessment process ,in line with personal recovery plans, including to service users who maybe on othercaseloads.
This might include : a. Individual or group therapeutic intervention b. Psychologicaltreatments such as CBT, DBT approaches , family interventions c.

To deliver a range of defined activities / interventions to improve the carers ability to support theservice user and to enable them in their relationship with the service To develop and maintain good partnership working with other services throughout all treatmentepisodes,
including regular liaison within Primary Health Care Team, inpatient and intensive services,day services, voluntary sector and with nominated carers / In collaboration with service users and carers, to be responsible for facilitating the development ofcomplex comprehensive crisis plans, rapid access plans, advance statements etc, involving otheragencies such as primary care etc where To personally build and lead others to build, hope inspiring relationships with service users, whichacknowledge the personal journey of each person, and focus on strengths and aspirations to allow thecreation of meaningful personal recovery To be responsible for maintaining own workload, as well as planning the workload of others, on a dayto day basis,
ensuring that time is prioritised effectively, utilising of electronic resources such asdiaries / To be responsible for the protection of individuals from abuse and harm in line with localsafeguarding policies and procedures, including working with others in the planning development andreview of protection plans, ensuring the appropriate sharing of Coordinating and undertaking protective interventions which maybe required in the protection ofothers, as outlined in the protection plan, taking individual action where Personally and leading others to collaboratively and sensitively work with individuals, with a range ofmental health needs to develop skills to manage their own health, in accordance with their personalrecovery plan, by actively promoting and using approaches which are affirming, build on strengths,identify past positive experiences and success, and use small steps to move towards the persons Undertake supervision for other colleagues within the team, covering for the manager andcolleagues as To maintain the single health and social care record, ensuring both paper and electronic records arekept up to date in accordance with professional and organisational Report and record within agreed timeframes, all activity relating to information reporting andperformance To participate in management, caseload and clinical supervision in accordance with trust To provide mentoring / training for others in relevant practice areas, according to professionalrequirements, taking a collaborative approach to practice development and evidence-based care.

Person Specification
Education and Qualification
Essential
- Registered as an OT, SW or RMN (parts 3 or 13 of NMC register)
- Formal post-graduate diploma level qualification in a relevant field
Experience and Knowledge
Essential
- Significant post-qualifying clinical experience obtained in a variety of settings
- Experience of working with women experiencing perinatal mental health needs / knowledge of demographic risk factors for this client group
- Clear record of ongoing continuing professional development
- Demonstrates experience as a Care coordinator or equivalent role of complex cases
- Demonstrates a detailed understanding in the theory and practice of Recovery Principles
Desirable
Experience or delivery of training / teaching within MDT / external organisations
Skills and Abilities
Essential
- Able to articulate an advanced and detailed understanding of the relevant legal Frameworks
- Demonstrates a clear knowledge and understanding of the issues and statutory processes involved in caring for vulnerable people
