Job summary

To be an active member of the clinical team, providing a comprehensive therapy service to residents of Barnet referred into the service.

To work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients / clients and maintaining professional documentation.

To work as part of a multidisciplinary team to facilitate positive rehab outcomes and timely, coordinated discharge planning.

To build and maintain key relationships with referrers to the service,





to facilitate appropriate referrals and an understanding of the role of the Physiotherapist and the Multi-professional team

To fully support initiatives to reduce length of stay, including key working.

Main duties of the job

To develop and implement treatment plans for clients.

To be responsible for organising and prioritising own caseload to meet service needs and to communicate with your supervisor if the service needs cannot be met.

To undertake comprehensive assessments and treatment plans of clients. These clients will present with stroke and other neurological conditions, an exacerbation of a frailty related condition and will likely have multiple co-morbidities including, musculoskeletal conditions, osteoporosis, learning disability and / or mental health diagnosis.

To use advanced clinical reasoning skills to formulate and deliver individual and group treatment programmes.

Job description

Job responsibilities

To continuously evaluate own practice ensuring it is up-to-date and evidence based, complying with Trust and Professional clinical guidelines.

To manage clinical risk within own patient case load to meet service needs and to communicate with your supervisor if the service needs cannot be met.

Travel to clients homes taking necessary assessment and treatment equipment as required.

Be responsible for the safe storage and safe moving and handling of equipment.

Work as a lone practitioner, within the guidelines of the Lone Working Policy,





when performing home visitsIdentify clients who need support from outside agencies, using clinical reasoning and analysis skills. .

Prescribe appropriate equipment, following local procedures, to clients in order to maximise their independence.

Person Specification

Education

Essential

- Diploma / degree in Physiotherapy
- Current HCPC registration
- Evidence of up to date CPD portfolio
- Involvement in Appraisal Scheme
- Attendance at specialist short courses or equivalent CPD activities within the specialist areas of Adult Rehabilitation, Neurology, Orthopaedics, Musculoskeletal Outpatients, Older People Rehabilitation
- Member of Professional Body (CSP)

Desirable

- Experience post registration as a Band 5 physiotherapist






- Experience working at a Junior level in a wide range of specialised clinical areas including outpatients, neurology, rehabilitation, orthopedics and respiratory
- Experience working in neuro / stroke rehab
- Experience working in the NHS

Experience

Essential
