Job summary

The post holder will work as part of the Discharge Teams to facilitate transfers from hospital and community beds, working in partnership with colleagues across health and social care to reduce length of stay and improve outcomes for patients and carers.

We are looking for a highly motivated and enthusiastic clinician who will provide professional and clinical support within the Discharge Teams and community beds as the service requires, which can include the Transfer of Care Hub, the Transfer of Care Team, Discharge to Assess, and the Intermediate Care at Home Team







This is a really exciting time to join our team to become part of our integrated service as we have many new and exciting initiatives to challenge and develop our staff .

The candidate should demonstrate strong communication / negotiating skills and be able to work closely with members of the MDT and other stake holders to facilitate timely and safe discharges to aid patient flow within the hospital and the community beds.

You will provide clinical leadership, expertise and advice to staff pertaining to all matters of discharge, quality and safe patient flow.

Main duties of the job

The key aims are as follows :

- Excellent communication and IT skills are required to triage patients to the right team and services.
- Delivering a consistent, co-ordinated approach to assist patients to identify potential needs at the point of discharge to avoid unnecessary delays in transfer of care
- Working collaboratively with external and internal partners for the benefit of the patients to ensure smooth transition from hospital and reduce risk of readmission
- Transparently providing information and expert advice to patients,





carers and other professionals
- Reporting all delayed transfers of care, sitrep reportable and non sitrep reportable, as per the local delay standards and criteria

Operating hours are 08 : 00-22 : 00 seven days a week.

Job description

Job responsibilities

- Undertake holistic patient assessment and triage appropriately following the agreed SOP
- Work in partnership with health, social and voluntary agencies to facilitate a seamless pathway of care for patients
- Produce and maintain accurate records both written and electronic in a timely manner.
- To abide by all relevant policies including Information governance, confidentiality, and data protection.
- Understand own responsibilities and accountability in the delivery and planning of care to patients,





working within own competence and professional code of conduct.
- To communicate with patients, carers and staff as part of effective patient care.
- To lead the team in the absence of the Team Leads
- To supervise, lead, inform and delegate staff as appropriate to the needs of the service.
- Applies analytical skills to assess and interpret referrals into the service, and in conjunction with the wider team plan the patients pathway.
- Demonstrates a high level of intuitive and creative problem-solving skills to facilitate patient centred care; rationalising information to promote patient safety by using a high level of communication and negotiation skills to affect the outcome.
- To undertake Professional Development Reviews with lower bands of staff in the team after appropriate training.






- To participate in the investigation of complaints and safeguarding concerns in accordance with the level of experience.
- To ensure all accidents, complaints and unusual incidents are reported in accordance with policy.
- To analyse workloads and review work patterns of the team as required.

If successful this vacancy will require an enhanced DBS check at a cost of £ This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
