Job summary

We have an exciting opportunity to be part of a new service as a Band 4 Clinical Support East London Intensive Support Team provides a 7 day a week urgent and intensive community response for people with Learning Disabilities with additional complex mental health needs and / or behaviours that services perceive to be challenging, that result in the person posing a risk to themselves or others.

The service operates across the inner east London boroughs of Tower Hamlets, Newham and City and Hackney. The team will be based at our Tower Hamlets site at Mile End post holder will assist other professionals in the IST in providing assessment, care, treatment and discharge planning.







This will be achieved through close collaboration with the person, carers, colleagues and other service providers. This may include supporting people during admissions to acute mental health post holder will work directly with service users to develop a rapport based upon respect, kindness and compassion that builds the basis of a professional relationship.

You will have a significant role in supporting service users to engage with their agreed Care Plans, positively promoting independent living and note that the closing date of this advert is the 1st of July or when a sufficient number of qualified applicants has been received.

Main duties of the job

- To support theIntensive Supportteamin providing assessments, care and treatment for People with learning Disability, Mental health issues and behaviours which challenge in the community under direct or indirect supervision of the team.
- To use skills in working with people with learning disabilities who are experiencing a Crisis to maintain, promote and improve health.






- To provide advice and consultation to colleagues supporting Autistic Adults who do not have Learning Disabilities.
- To facilitate access to health services through collaboration and partnership with primary and secondary health, and other relevant services.
- Developing working relationships with clients, provider organisations, GPs, local community resources.
- Support in the delivery of generic health tasks such as the completion of Hospital Passports, Health Action Plans, monitoring of mental health and behaviours which may challenge.
- Prepare clients and carers for health appointments to enhance their understanding to enable them to actively participate in their own health care.






- To contribute to shaping local health services in the way they are organised and run to promote an inclusive service for people with learning disabilities.

Person Specification

Education / Qualification / Training

Essential

NVQ Level 3 qualification to diploma equivalent level or above

Desirable

Qualification in health or social care related subject

Experience

Essential

- Experience of working with people who have a disability and / or physical / mental health / sensory difficulties
- Experience of using Excel software for data recording and reporting
- Experience of team working
- Experience of co-facilitating and presenting information to groups
- Experience of working with people who have a learning disability, mental health issues and behaviours which may challenge
- Experience of working with people who have communication difficulties or are non-verbal






- Experience of working with paid and unpaid carers

Desirable

- Experience of working in a community-based health or social care setting
- Experience of gathering health-related data and information
- oExperience of working with people whose first language is not English and of working through interpreters

Knowledge and Skills

Essential

- Understanding of what a learning disability is and the impact it may have upon someone on a day to day basis
- Ability to adapt communication style to communicate effectively with those who have limited verbal communication skills
- Good interpersonal and written communication skills
- Competence in the use of IT software - Microsoft Word, Excel, PowerPoint, Outlook, Teams; Zoom
- To be willing to learn new clinical skills

- Knowledge and understanding of cultural diversity the
- Ability to plan, organize and prioritise workload, to problem-solve and to work to deadlines
- Coaching / motivational interviewing skills

Essential

- Ability and willingness to work across different sites and within the community ( client's homes, day services).
- Ability to recognise boundaries of own skill level and seek help as appropriate
- Respectful and professional approach to service users, families, carers, colleagues and other professionals. Willing to negotiate and can handle confrontation effectively and professionally.
