Job summary

We are thrilled to present an exceptional fixed-term opportunity as a Lead Wellbeing Practitioner within the Southeast London Integrated Care System.

We are actively seeking an experienced and autonomous individual to join our LD and Autism (LDA) STOMP team, providing holistic care and positive support for STOMP-eligible service users.

Your role will be crucial in reducing unnecessary psychotropic drugs in primary care and community settings and making a positive impact on the quality of life of individuals with learning disabilities and autism.

As the ideal candidate, you should be self-motivated and possess extensive experience working with LDA service users in the community or primary care settings.







Your expertise in service user case management and supporting NHS England's STOMP and STAMP priorities will be essential.

Additionally, you should have a track record of general behaviour support and PBS for LDA service users, as well as managing caseloads and engaging with multi-disciplinary clinicians, service users, and carers.

A secondment opportunity is available for current SLaM and Oxleas employees provided you have your line manager's approval.

Responsibilities for this role include :







Taking operational responsibility for implementing postive support strategies and interventions for STOMP-eligible service users across all six boroughs in SEL ICS.

Conducting timely behaviour and risk assessments for service users referred to the STOMP pathway, facilitating care planning and support them during the STOMP process.

Implementing behaviour support for LDA service users before and during the optimisation of psychotropic drugs.

Engaging with key team members, stakeholders, LDA service users, their carers, and GPs to facilitate PBS assessment and implementation.

Conducting community visits to service users while receiving clinical supervision from the lead clinical psychologist.

Collaborating and receiving peer support from both the Community Learning Disability Team (CLDT) - Oxleas and Mental Health in London Disability Team (MHLD) South London and Maudsley (SLaM) ,





as well as the STOMP / STAMP team from South East London Integrated Care Board (SEL ICB)

Person Specification

Qualifications

Essential

- Bachelor's Degree followed by PG Diploma in Applied Behaviour Analysis, or Analysis and Intervention in Intellectual Disabilities, or equivalent PG Diploma
- Clear evidence of continued professional development.

Experience

Essential

- Experience of working with people with mental health, learning disability and / or autism who display behaviours' that challenge
- Experience of undertaking functional analysis and developing positive behaviour support plans






- Experience of working as part of a multi-disciplinary team
- Experience of working in community services.

Desirable

Experience of a leadership role within PBS

Knowledge / Skills

Essential

- A sound knowledge and understanding of positive behavioural support models and its theories and practices
- Excellent communication and interpersonal skills, including an ability to communicate complex information to clients and a variety of professionals
- Ability to manage verbal and physical aggression directed at self, including use of breakaway techniques and personal safety procedures
