Job summary

We are looking for a Consultant Advanced Clinical Practitioner to join our East Kent HCP Frailty / Home Treatment Service.

You will will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to older people within a variety of healthcare settings.

S / he will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of the older person.

Main duties of the job

You will lead and monitor improvements to quality of care through clinicalaudit, evidence based practice, clinical supervision,





teaching and supportingprofessional colleagues and the provision of skilled professional leadership.

You will lead the implementation of relevant clinical guidelines into practice and contribute tothe further development of the evidence base through research and audit.

You will lead the development of non-medical led models of care acrossEmergency departments, inpatient wards, intermediate care, care homes and community teams in collaboration with multi-professional colleagues.

You will play a crucial role in improving patient flow through timely assessment, referral, responsive intervention, anticipatory care planning and optimal discharge coordination across seven days

With this in mind as part of the selection process for this position you will be asked to complete two other activities in addition to the competency-based interview that you will invited to.

This extended selection process is designed to assess your management style and the specific details of the exercises will be given if you are successfully shortlisted.

Person Specification

QUALIFICATIONS

Essential

o tFirst level Registration with NMC, HPCP or GPHC o tMinimum of 5 years post registration experience,





Experience at band 8a or above o tIs an Advanced Clinical Practice MSc accredited by the Centre for Advancing Practice OR Successful recognition with the Centre of Advancing Practice via the ePortfolio (Supported) Route to demonstrate educational equivalence that all the capabilities of advanced level practice at master's level (level 7) have been met.

o tMaintained a portfolio of ongoing evidence or work based learning. o tTeaching qualification o tResearch qualification o tLeadership o tNon- medical Prescribing

Desirable

o tPsychology or counselling experience and training / education

EXPERIENCE

Essential

o tExtensive clinical experience in the care and treatment of people with frailty (including long term conditions and dementia care experience)





o tAdvanced skills in history taking and physical assessment of frail people o tIndependent prescribing experience o tExperience of advanced specialist interventions in the assessment and management of older patients o tExperience of teaching and education provision o tExtensive experience in report writing o tExperience of planning, implementation and delivery of educational programmes relating to long term conditions o tExperience of working across organisational boundaries with partners (commissioning, social care and education providers) o tAdvanced IT skills o tExperience with evidence of audit, research and innovation applied in practice o tExperience of strategy development and implementation

o tExtensive experience of presenting report / education / information to public forums o tIR(ME)R training or equivalent
