Job summary

The South London Partnership are looking to recruit an 8a CAMHS Case Manager - Mental Health. The post holder will work across a number of Providers both NHS and independent sector, ensuring CAMHS patients receive the best quality care as near to home as possible.

The successful candidate will be responsible for the quality assurance of a number of wards in South London. The case manager works closely with the Provider to ensure high quality care is maintained.

The successful candidate will interface with a range of other stakeholders. You will work with other Case Managers in the management including particularly complex cases and delayed discharges including Transforming Care.







You will also work in collaboration with services within the hub to manage relationships with ICS colleagues. This role requires you to work in collaboration with local authorities, third sector and other statutory bodies in the management of effective usage of the specialised mental health services.

Main duties of the job

As CAMHS Case Manager you will provide clinical support to the specialised commissioners in relation to the defined population of South London which will include projected mapping of future service use and planning in terms of future service needs.

You will work as part of a Case Management Team supporting managers and the MDT to ensure safe and effective care pathways are in place for all service users.

You will work with Providers and offer quality oversight to ensure commissioned services meet the required standards.





You will work closely with partners to embed the principles of prevention, early intervention, community-based provision and least restrictive practice.

You will maintain effective, open and clear communication with Providers, NHSE, CCGs and Local Authority to ensure strong joint working.

Your role will work across the Transforming Care Programme and Mental Health Programme. You will support the delivery of the Transforming Care Programme by working with colleagues within Integrated Care Board, other Provider Collaboratives, NHSE, Local Authority SLP ward staff.

You will responsible to support both Mental health and Transforming care cohort in relation to appropriate admission and smooth discharge.

Key Responsibilities :

Improving quality and outcomes

- To provide clinical assurance of the quality of the care provided by specialised services within the South London Provider Collaborative and Commissioning Hub (PCaDH)
- Supporting the South London Partnership (SLP) team in the assurance of specialised services compliance to applicable specification and guidance.
- The post-holder will provide the link with Clinical Commissioning Groups and their commissioned services within the hub to ensure there is a co-ordinated approach to care and discharge planning, which results in robust and effective care packages.
- Managing individual patient pathways to ensure high quality services and care planning outcomes are achieved.






- To take action in respect of Safeguarding Policies and Procedures as required

Enabling patient and public involvement

- To act as a champion for patients and their interests and involve the public and patients in the policy development and decision-making of the SLP.
- To ensure all public and patient contact with the office is of highest professional standard.
- To embed patient and public involvement within South London at all levels of decision making.

Partnership and cross boundary working

- To work across multiple providers within the NHS and Independent Sector
- To work with other Case Managers in the management of particularly complex cases and delayed discharges (including Transforming Care)
- To work in collaboration with services within the hub to manage relationships with CCG commissioned providers that provide the referral and aftercare components of the pathway.
- To work in collaboration with local authorities, third sector and other statutory bodies such as MAPPA, Police, HMPPS, MOJ and CJS in the management of effective usage of the specialised mental health services

Using insight and evidence for improvement

- To ensure that safe and effective care pathways are in place to facilitate the movement of service users through specialised services, including the identification and reporting of deficits within service provision

- Ensure case management systems and record keeping are maintained and kept up to date

- To ensure the health, safety and wellbeing of all staff within the SLP

Project Management

- Lead the team in the delivery of project plans, allocating tasks as appropriate, identifying risks, issues and dependencies, considering best practice and current options and ultimately making decisions in the best interest of the project.
- Be responsible for a high standard of work supporting the delivery of projects on time, to quality standards and in a cost effective manner.
- Maintain the project initiation document and associated plans with regular team meetings to monitor progress and resources.
- Demonstrate effective stakeholder management.
- Support other Case Manager / Project Managers as and when required.
- Take into account the impact of any change the projects will have on the business and oversee handover of any products to ensure full ownership and buy-in within the business Advocate the projects at senior and executive levels and ensure active engagement and sponsorship within NHS as a whole.
- Ensure that the projects maintain business focus, have clear authority and that the context, including risks, is actively managed in alignment with the strategic priorities of SLP.

Financial and Physical Resources

- Act in a way that is compliant with Standing Orders and Standing Financial Instructions in the discharge of budget management responsibilities, monitor expenditure on a regular basis.
- Identify products, equipment, services and facilities for assigned activities, achieving stakeholder buy-in as required.

Responsible for making recommendations, providing advice and able to prepare strategic reports / briefings for the SLP and relevant forums as required

People Management

- Forge close positive working relationships, in order to support an effective matrix approach to achieve SLP objectives

- Managing third parties (such as consultants / interims) to ensure deliverables are met in a timely manner and within budget.

Information Management

- Drafting reports summarising status on issues, appraising outcomes, and providing progress reports as required
- Collate as required, a range of information and lead appropriate analysis to develop robust business cases and contribute to project outcomes
- Analyse, interpret and present data to highlight issues, risks and support decision making

Policy and Service Development

- Responsible for proposing and drafting changes, implementation and interpretation to policies, guidelines and any other documents which may impact service.
- Proposes changes to own function making recommendations for other service delivery.

- Plan, develop and evaluate methods and processes for gathering, analysing, interpreting and presenting data and information
- Co-ordinating Research and Development initiatives, delegating as appropriate

Person Specification

Qualifications

Essential

- Registered Mental Health / Learning Disability Nurse or other registered health or social care professional
- Possess a relevant additional professional qualification, masters level or equivalent level of experience of working at a senior level in specialist area
- Member of relevant professional body Registered Mental Health / Learning Disability Nurse or other registered health or social care professional

Desirable

Further training or significant experience in project management, financial management or supporting change management processes

Experience

Essential

- Experience of working within a CAMHS setting either in-patient or community
- In depth understanding of a pathways, treatment and care for people with Learning Disabilities, ASD and Mental Health issues, including CAMHS services
- Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement
- Should have an appreciation of the relationship between the South London Partnership, NHS England and individual provider and commissioning organisations
- Knowledge of legal issues pertaining to the specialist service, including MHA 1983
