We are seeking a highly motivatednurse with palliative care skills, knowledge and experience who has teachingand learning at the heart of their practice.

You will have a good workingknowledge of the evidence that supports development and quality improvement.The role will support existing and new assessors and supervisors, clinical studentsand all levels of staff as well as undertake specific project work.







You will bepart of the education team and work closely with clinicians and be supported bythe Head of Professional Practice & Education.

A drive for change and a creativeand focused approach will be required to achieve organisational priorities for practicedevelopment.

Main duties of the job

To be a role model for all staff groups, demonstrating a professionaland pro-active approach, providing advice, guidance, and support to staff inclinical practice.

In collaboration with senior clinical post holders,actively participate in service improvement by facilitating an understanding ofevidence andmanagement, and work alongside colleagues to support them in their role ofdeveloping staff.







This will require application of specialist knowledge intopractice in a range of contexts, development of education developmentprogrammes for learners and mentors and training of specific skills inpalliative and end of life care.

To lead projects that streamline practiceprocesses, in accordance with guidance and evidence, to ensure policies areupdated and implemented using a systematic approach.

We also have the hospice Pepperell Education Centre that has an excellent reputation for delivering high quality education programmes and study days.

JOB TITLE : Practice Education Facilitator

GRADE : 6b

HOURS OF DUTY : (full time or job share considered)

REPORTS TO : Head of Professional Practice & Education

KEY RELATIONSHIPS : Multi-professional team of all levels, service users, internal and external care providers, higher education institutions (HEIs), volunteers, education team

RESPONSIBLE FOR :





This role does not have line management responsibility.

MAINDUTIES AND RESPONSIBILITIES

LEADERSHIP AND TEAMWORKING :

Support the nursing and multi-professionalteam to attain and develop high standards of care through evidence-basedpractice.

Role model a supportive, safe environment in which to practice, and develop skills, confidenceand maturity in specialist palliative and end of life care, whilst recognisingthe emotional demands of this speciality.

Participate in the development, updating andimplementation of clinical policies and procedures, and integrate this withinvarious aspects of the role, including teaching.

Participate in strategic hospice development as part of theeducation team.







Work with the Practice & Quality Improvement Lead to highlightrisks and plan mitigating actions / processes, and utilise in the plans forpractice education and training.

Keep appropriate education and training records inconjunction with teamadministrators, in line with SFH policy.

Promote an open, supportive culture that facilitates learning from incidents to ensure patient safety, and proactively lead to enable teamparticipation in decision making and implementation.

Undertake specific project work, supported by the educationteam colleagues, to bring together stakeholders, clinical guidelines,evidence-based practice, specific equipment requirements, and policies andprocedures to develop processes for implementation, including training andsupport.







Contributeto reporting of practice education activity as part of the clinical governancestructure.

Assessand benchmark clinical education practice, and support mentors and students inrelation to placements, competence and fitness for practice, followingguidelines from several HEIs.

Leadannual education audits with HEIs and disseminate outcomes and learning, andimplement any changes as part of team practices.

Workwith digital education and academic teaching staff to ensure new learning andprocesses are integrated into practice development.

CLINICAL PRACTICE

Actively promote and support an evidence-basedpractice approach in clinical practice as part of robust practice development.

Participate in the quality and clinical auditprogramme as guided by the Practice & Quality Improvement Lead.





Identifyand promote areas of practice that require auditing and monitoring, and supportskill development to enable staff involvement.

Workalongside colleagues in clinical practice to enhance skill development,including students on placement and those in mentorship roles, providing bestpractice advice to support care delivery and governance.

Maintain all records and associated documentationin accordance with Saint Francis Hospice policies, utilising a range of ICTskills, equipment, and systems.

Promote and support the resilience based reflectivepractice groups, as part of wellbeing, communication skills development andcultural development.

Provideon-going support to student nurse Supervisors and Assessors (mentors), as wellsupport to other student groups on placement.

This will include pastoralsupport to mentors on the occasions whenfeedbackon performance and failing students is required.

This will involve requireclear liaison with universities and following of processes.

Supervisethe induction of new staff with regard to standards of competence andprofessional development, including preceptorship for newly qualified nurses.

Supportthe wider clinical team and other members of staff in development of clinicaland holistic skills, by using a combination of working alongside, grouptraining and teaching.

COMMUNICATION

Ensure effective communication with allstakeholders, including NHS colleagues in community and acute services, HEIs,voluntary services and the hospice sector.

Ensure communication and collaboration with SFHcolleagues in to inform practice development needs and planning of activity toaddress them.

Communicate educational and practice information orprocesses with HEI and practice partners, following any change at Saint FrancisHospice.

Developprofessional relationships through effective communication and use of a varietyof IT systems (some external to SFH) to establish clear information aboutstudent placements.

Organise practice-based placements, including objectives,outcomes, assessments, and financial detail as appropriate, and support withlearning and development programmes as part of the placement.

This will applyacross a range of disciplines, including nursing and paramedics.

EDUCATION OF SELF AND OTHERS

Following liaison with HEI partners, allocatestudent placements, assessors and supervisors (mentors) and support theprocesses including designing and delivering of training.

Role model an awareness of professionaldevelopments in nursing and palliative and end of life care and integrate intopractice development activity, including working in clinical practice.

Identify areas requiring staff development, forexample knowledge and skills, and participate in its delivery, liaising withcolleagues to enable implementation.

Participate, encourage and empower team members todevelop their own knowledge and skills and signpost to opportunities forfurther learning and skill acquisition.

Signpostthe development of material for conference presentations asappropriate,and show willingness to present own work and encourage others to develop skillsin abstract writing / academic skills.

Contribute to the planning, design and delivery of trainingand education, as part of wider education activity, alongside the educationteam in line with strategic priorities.

Develop and update competency assessments in accordancewith published guidelines and initiatives, and facilitate completion inconjunction with service managers.

Keepup to date with publications on practice development, and share any learningwith colleagues to assist with innovation and new approaches.

Contribute to review the Service ImprovementGroups, in liaison with the Practice & Quality Improvement Lead, and thesupport skills development to undertake audits and update policies, and informclinical practice.

Contribute to the mandatory training, with a focuson clinical skills and / or knowledge that has a direct implication for clinicalpractice.

Ensure familiarity with new technology and ensuredigital capability, with support, to enable approaches to be part of practicedevelopment activity.

PROFESSIONAL RESPONSIBILITIES

Take responsibility for own professionaldevelopment, which will be monitored as part of the annual appraisal review andsupervision sessions.

Alwaysmaintain current registration with the NMC and adhere to the NMC Code ofConduct.

Person Specification

Qualifications

Essential

- Registered Nurse.
- Degree level qualification in Palliative Care or equivalent level of experience.
- Membership of the Nursing and Midwifery Council.

Desirable

- Training in audit / quality improvement or equivalent experience.
- Teaching qualification Diploma in Education and Training or equivalent experience.

- Proven presentation skills.
- Computer literate in word, PowerPoint and Excel. #
- Good working knowledge of relevant standards and guidelines in palliative care and research.
- Ability to apply effective communication to support care teams as they face challenges in team working and service delivery.
